【摘要】:脊髓损伤(spinal cord injury SCI)后患者生活质量严重受损,,造成难以恢复的终身残疾,后果严重,是医学领域的难题之一。随着SCI研究的不断深入,人们逐渐认识到减轻脊髓继发性损害是促进脊髓损伤后再生和进一步治疗的基础。嗅鞘细胞(olfactory ensheathing cell OECs)因其独特的细胞生物学特性成为了近年来脊髓损伤后移植治疗的热点。移植嗅鞘细胞能促进SCI后神经轴突的再生,但其效果不一,如何合理的进行移植,最大程度的发挥其作用,是值得关注的研究方向。SCI后移植细胞的目的是在脊髓两断端之间建立引导、促进神经纤维再生的桥梁。目前常用的移植方法是在脊髓损伤区两残端内植入嗅鞘细胞,而不是直接移植到两断端之间,其原因有:损伤区的恶劣环境不利于移植后嗅鞘细胞的存活繁殖;此外移植的嗅鞘细胞不能和两脊髓断端保持有效的接触可能影响其迁移,嗅鞘细胞不能有效发挥作用。SCI后损伤区的继发性病理变化不利于移植后嗅鞘细胞的生存,与此相关的因素众多,譬如:
[Abstract]:After spinal cord injury (spinal cord injury sci), the quality of life of the patients is seriously damaged, resulting in a life-long disability that is difficult to recover, and the consequences are serious, which is one of the difficult problems in the medical field. With the development of sci, it is gradually recognized that reducing the secondary injury of spinal cord is the basis of promoting regeneration and further treatment after spinal cord injury. Olfactory ensheathing cells (OECs) have become the focus of transplantation treatment after spinal cord injury in recent years because of their unique cellular biological characteristics. Transplantation of olfactory ensheathing cells can promote the regeneration of axons after sci, but the effect is different. The purpose of transplantation cells after sci is to establish a bridge between the two broken ends of spinal cord and promote the regeneration of nerve fibers. At present, the commonly used transplantation method is to implant olfactory ensheathing cells into the two stumps of spinal cord injury area, instead of transplanting directly between the two broken ends. The reasons are as follows: the bad environment in the injured area is not conducive to the survival and reproduction of olfactory ensheathing cells after transplantation; In addition, the transplanted olfactory ensheathing cells could not maintain effective contact with the two spinal cord broken ends, and the olfactory ensheathing cells could not play an effective role. The secondary pathological changes in the injured area after sci were not conducive to the survival of the olfactory ensheathing cells after transplantation.
