河南省示范区艾滋病防治人员随访管理能力现况调查

【摘要】:目的 了解河南省县/市、乡/镇、村三级定点医疗机构和疾病预防控制中心从事随访管理工作爱滋病防治人员能力现状,同时对随访管理能力测评结果进行分析,并探讨其影响因素,最后提出加强人员能力建设的策略与途径,旨在为进一步推动河南省艾滋病防治队伍随访管理能力建设提供科学依据和政策建议。 方法 利用文献回顾分析法对艾滋病防治随访管理人力资源现状进行回顾性分析。采用匿名式问卷调查法对河南省济源市、永城市、确山县、上蔡县、沈丘县五个项目示范县/市139名从事艾滋病随访管理工作人员进行问卷调查。采用半结构化访谈法对57名艾滋病随访管理工作人员进行定性访谈。 结果 1.通过问卷调查艾滋病随访管理工作人员139名,其中县/市级56人,乡/镇级30人,,村级53人;男性81人,女性58人;年龄以35~45岁为主;本科及以上学历22人,占15.8%;初级职称占44.6%。定性访谈57人,其中男性37人,女性20人;年龄以35~45岁为主;学历主要为中专及大专,分别占45.6%、35.1%;职称以初级为主,占38.6%;专业多为临床,占57.9%。 2.139名从事随访管理工作人员中没有编制者占35.3%;工作年数超过5年者占80.6%;人员在工作中需要加班的情况占92.0%;认为从业工作负荷大者占54.0%;调查对象认为“待遇低”是影响工作开展最主要的困难。 3.乡/镇级和村级机构人员接受随访相关知识培训的比例均超过90%;100%的受访对象认为培训不及时且次数较少;机构级别越低接受更高级别培训的机会越少,接受过国家级培训的人员比例仅为8.3%;人员最希望接受的培训内容包括艾滋病临床治疗知识、艾滋病宣传教育知识、心理沟通方法与技巧、艾滋病防治新进展等。 4.艾滋病防治人员随访管理能力测评平均得分27.78±5.50分,达标率为80.58%;八项能力测评得分最高的方面为专业知识技能(3.71±1.00分)。艾滋病随访管理能力的主要影响因素为:单位级别(OR=6.709;OR=8.348)、学历(OR=6.012)、编制情况(OR=4.252)、从事随访工作时间(OR=7.999;OR=5.382)和培训级别(OR=4.220)。 结论 1.河南省从事随访管理工作艾滋病防治人员基本专业知识技能具备但整体水平不高,综合能力有待进一步加强,需要通过优化人力资源配置进行改善。 2.人员工作任务繁重、工作压力大且待遇水平偏低,执业状况不容乐观,制约了随访队伍的能力建设与提高。 3.艾滋病随访相关专业知识技能培训开展不到位,建议有针对性地多开展高层次专业化培训以提高艾滋病防治人员从事随访管理工作的能力。
[Abstract]:Purpose. To understand the current situation of AIDS prevention and control personnel in the follow-up management of three level designated medical institutions and disease prevention and control centers in counties / cities, townships / towns and villages in Henan Province, and to analyze the results of the assessment of follow-up management ability. Finally, the strategies and approaches of strengthening personnel capacity construction are put forward to provide scientific basis and policy suggestions for further promoting the follow-up management capacity building of AIDS prevention and treatment teams in Henan Province. Method. The present situation of human resources in follow-up management of AIDS prevention and treatment was analyzed retrospectively by literature retrospective analysis. The anonymous questionnaire survey was used in Jiyuan, Yongcheng, Queshan and Shangcai counties in Henan Province. A questionnaire survey was conducted among 139 AIDS follow-up management staff in five project demonstration counties in Shenqiu County, and 57 AIDS follow-up management staff members were interviewed qualitatively by semi-structured interview method. Results. 1. A questionnaire survey was conducted among 139 AIDS management staff, including 56 at the county / city level, 30 at the township / town level and 53 at the village level; 81 men and 58 women; the majority of them were 35 to 45 years of age; 22 had bachelor's degrees or above. Qualitative interview 57, male 37, female 20; age 35 ~ 45 years old; academic background is mainly technical secondary school and junior college, accounting for 45.6% 35. 1 respectively; professional title is primary, 38.6%; major is clinical, accounting for 57.9%. 2.139 follow-up management staff members who did not have a staff establishment accounted for 35.3x; those who worked for more than five years accounted for 80.6; the number of people who needed overtime in their work accounted for 92.0; those who considered that the workload was heavy accounted for 54.0; and the respondents considered that "the pay was low". The main difficulty affecting the performance of the work. 3. At the township / township and village levels, more than 90 per cent of the participants surveyed thought that the training was not timely and less frequent; the lower the institutional level, the less opportunities for higher levels of training, The proportion of personnel who have received training at the national level is only 8.3. The training content most desired by the staff includes AIDS clinical treatment knowledge, AIDS propaganda and education knowledge, psychological communication methods and skills, new progress in AIDS prevention and treatment, and so on. 4. The average score of management ability assessment for AIDS prevention and treatment personnel was 27.78 卤5.50. The achievement rate was 80.58, the highest score of the eight abilities was 3.71 卤1.00 scores of professional knowledge and skills. The main factors influencing the ability of AIDS follow-up management were as follows: 6.709 units, 6.012, 4.252, 7.999, 5.382, and 4.220, respectively. Conclusion. 1. The basic professional knowledge and skills of HIV / AIDS prevention and control personnel in the follow-up management work in Henan Province have been possessed, but the overall level is not high, the comprehensive ability needs to be further strengthened, which needs to be improved by optimizing the allocation of human resources. 2. The heavy work task, the heavy work pressure and the low salary level, the practice condition is not optimistic, which restricts the ability construction and improvement of the follow-up team. 3. AIDS follow-up related professional knowledge and skills training is not in place, it is suggested that more high-level professional training should be carried out to improve the ability of AIDS prevention and treatment personnel to engage in follow-up management work.
