慢性乙肝患者乙肝病毒数量与糖代谢常用临床指标相关性分析

【摘要】：目的:研究乙型肝炎病毒与慢性乙型肝炎患者临床常用血糖代谢指标的相关性。方法：选取2011年9月至2013年10月期间在新疆医科大学第五附属医院内分泌科就诊的慢性乙型肝炎病史小于5年的新发糖尿病患者128人,对其乙肝病毒DNA定量数量与临床常用血糖代谢指标进行岭回归分析。结果：排除混杂因素的影响后行岭回归提示乙肝病毒DNA数量与血糖水平(r=0.297,P0.001；r=0.156,P0.001; r=0.656,P0.001；r0.432,P0.001；r=0.649,P0.001).胰岛素水平(r=0.305,P0.001；r=0.476,P0.001；r=0.395,P0.001；r=0.386,P0.001；r=0.561,P0.001).C肽水平(r=0.251,P0.001；r=0.229,P0.001；r=0.254,P0.001；r=0.075,P0.001； r=0.252,P=0.010)、胰岛素抵抗指数(r=0.187,P0.001)、胰岛p细胞功能指数(r=0.517,P0.001)呈正性相关,与胰岛素敏感指数(r=-0.159,P0.001)、胰岛素作用指数(r=-0.140,P0.001)呈负性相关。结论：随着乙肝病数量的升高,血糖水平呈上升趋势,刺激胰岛β细胞大量分泌胰岛素,胰岛素抵抗加重,降低胰岛素敏感性,影响胰岛素的生理作用,从而引起或进一步加重血糖代谢紊乱程度。
[Abstract]:Objective: to study the correlation between hepatitis B virus (HBV) and blood glucose metabolism in patients with chronic hepatitis B. methods: from September 2011 to October 2013, we visited the Department of Endocrinology, 5th affiliated Hospital of Xinjiang Medical University. 128 new diabetic patients with chronic hepatitis B with a history of less than 5 years, The quantitative quantity of hepatitis B virus (DNA) and blood glucose metabolism index were analyzed by ridge regression analysis. Results: the results showed that the quantity of hepatitis B virus DNA and blood glucose level were 0.297g P 0.001 P 0.001 P 0.001; 0.656U P 0.001r0.432P 0.001r0.649p 0.001P 0.001.The insulin level was 0.305p 0.476p 0.395p 0.395P 0.001p 0.001 P 0.001P 0.001P 0.001P 0.001p 0.561p 0.561p 0.001p ~ 0.561p ~ 0.561p ~ (0.001) P ~ (0.001) P ~ (0.001) P ~ (?) ~ P ~ (?) ~ (?) ~ (?) ~ (?) ~ (?) _ _ _. There was a positive correlation between the levels of 0.251 (0.251) and 0.229 (P 0.001), 0.254 (P 0.001), 0.075 (P 0.001), 0.010 (0.252), 0.187 (P 0.001) and r0.517 (P 0.001) of the islet cell function index (P0. 017), the insulin resistance index (0. 187) and the islet P cell function index (r0. 517) were positively correlated with that of the pancreatic islet P cell function index (P0. 001), the insulin resistance index was 0. 187%, and the insulin resistance index was 0. There was a negative correlation between the insulin sensitivity index and insulin sensitivity index (r-0.159U, P 0.001). Conclusion: with the increase of hepatitis B disease, the level of blood sugar is increasing, which stimulates the secretion of insulin by 尾 -cells and increases insulin resistance. Reduce insulin sensitivity, affect the physiological effect of insulin, and cause or further aggravate the disorder of blood glucose metabolism.
