病毒性脑炎急性期癫痫发作与血清及脑脊液中IL-1β、IL-6及TNF-α表达水平的关系研究

【摘要】:目的:探讨病毒性脑炎急性期出现癫痫发作与血清及脑脊液中白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)水平之间的关系。方法:1.研究对象收集2016年3月-2017年1月就诊于山西医科大学第一附属医院神经内科住院的30例病毒性脑炎急性期患者,根据有无癫痫发作将其分为两组,A组为病毒性脑炎急性期不出现的癫痫发作患者,B组为病毒性脑炎急性期出现的癫痫发作患者。选取非病毒性脑炎性癫痫患者10例为C组,D组为10例需脊髓麻醉并排除感染及炎症的非癫痫患者(对照组)。2.标本采集在征得患者或家属的同意后,采集各组患者的血液及脑脊液,3000rpm离心10分钟后,分别提取上清液于1.5ml于EP管中,置于-80℃冰箱中冻存备用。3.血清及脑脊液IL-1β、IL-6及TNF-α浓度水平测定采用夹心法酶联免疫吸附试验(enzyme-linked immunosorbent assay ELISA)方法测定血清及脑脊液IL-1β、IL-6及TNF-α浓度水平。4.数据处理:用SPSS19.0统计软件进行统计学分析,所有数据用均数±标准差((?)±s)表示,各组数据采用单因素方差分析,组间比较采用用LSD方法,以P0.05表示有统计学意义。结果:1.各组血清中IL-1β、IL-6及TNF-α水平比较与对照组相比,A组与B组血清中IL-1β、IL-6及TNF-α水平明显升高,差异有统计学意义(P0.05);C组血清中IL-1β浓度水平较对照组略高,差别无统计学意义(P0.05),而C组血清中IL-6和TNF-α水平较对照组略低,差别无统计学意义(P0.05)。与C组相比,A组与B组血清中IL-1β、IL-6及TNF-α水平明显升高,差别有统计学意义(P0.05)。与A组相比,B组血清中IL-1β、IL-6及TNF-α水平显著升高,差别具有统计学意义(P0.05),提示病毒性脑炎急性期血清中IL-1β、IL-6及TNF-α水平与急性期癫痫发作有关。2.各组脑脊液中IL-1β、IL-6及TNF-α水平比较与对照组相比,A组与B组脑脊液中IL-1β、IL-6及TNF-α水平明显升高,差别具有统计学意义(P0.05);C组脑脊液中IL-6水平较对照组略高,差别无统计学意义(P0.05),而C组脑脊液中IL-1β和TNF-α水平较对照组略低,差别无统计学意义(P0.05)。与C组相比,A组与B组脑脊液中IL-1β、IL-6及TNF-α水平明显升高,差别有统计学意义(P0.05);与A组相比,B组脑脊液中IL-1β、IL-6及TNF-α水平显著升高,差别具有统计学意义(P0.05),提示病毒性脑炎急性期脑脊液中IL-1β、IL-6及TNF-α水平与急性期癫痫发作有关。结论:1.病毒性脑炎急性期癫痫发作患者血清中IL-1β、IL-6及TNF-α水平明显高于病毒性脑炎急性期无癫痫发作患者血清中IL-1β、IL-6及TNF-α水平,提示病毒性脑炎急性期癫痫发作与血清中IL-1β、IL-6及TNF-α水平升高有关。2.病毒性脑炎急性期癫痫发作患者脑脊液中IL-1β、IL-6及TNF-α水平显著高于病毒性脑炎急性期无癫痫发作患者脑脊液中IL-1β、IL-6及TNF-α水平,提示病毒性脑炎急性期癫痫发作与脑脊液中IL-1β、IL-6及TNF-α水平升高有关。
[Abstract]:Objective: To investigate the relationship between epileptic seizures in acute stage of viral encephalitis with interleukin -1 beta (IL-1 beta), interleukin -6 (IL-6) and tumor necrosis factor - alpha (TNF- alpha) in serum and cerebrospinal fluid. Methods: 1. subjects were collected in the First Affiliated Hospital of Shanxi Medical University in January March 2016 and were hospitalized in the Department of Neurology. The 30 patients with acute viral encephalitis were divided into two groups according to the seizures, the A group was the non epileptic seizures in the acute stage of viral encephalitis, and the B group was the epileptic seizures in the acute stage of viral encephalitis. 10 cases of non viral encephalitis epilepsies were selected as C group, and 10 patients in group D needed spinal anesthesia and excluded infection. The.2. specimen of the non epileptic patients (control group) and the non epileptic patients (control group) collected the blood and cerebrospinal fluid of each group after receiving the consent of the patients or family members. After 3000rpm centrifugation for 10 minutes, the supernatant was extracted from 1.5ml in the EP tube, and the concentration level of.3. serum and cerebrospinal fluid IL-1 beta, IL-6 and TNF- alpha concentration was measured in -80 centigrade refrigerator. Enzyme-linked immunosorbent assay ELISA method was used to determine the serum and cerebrospinal fluid IL-1 beta, IL-6 and TNF- alpha concentration level.4. data processing: statistical analysis was performed with SPSS19.0 statistical software. All data were expressed with mean mean + standard deviation ((?) + s). The data of each group were analyzed by single factor variance and compared between groups. In 1. groups, the levels of IL-1 beta, IL-6 and TNF- alpha in serum were compared with those in the control group, and the levels of IL-1 beta, IL-6 and TNF- a in the serum of A and B groups were significantly higher than those in the control group. The difference was statistically significant (P0.05), and there was no significant difference in the level of serum beta concentration in the serum of the C group, and there was no significant difference in serum levels of IL-1 beta, IL-6 and TNF- a in the serum of the B group. The levels of serum IL-6 and TNF- alpha in serum were slightly lower than those in the control group (P0.05). Compared with the C group, the levels of IL-1 beta, IL-6 and TNF- alpha in the serum of A and B groups increased significantly (P0.05). Compared with the A group, the levels of the serum in the B group were significantly higher than those in the A group, and the difference was statistically significant. The levels of IL-1 beta, IL-6 and TNF- alpha in the serum of acute toxic encephalitis were compared with the control group, the levels of IL-1 beta, IL-6 and TNF- alpha in the cerebrospinal fluid of.2. groups were compared with those in the control group. The levels of IL-1 beta, IL-6 and TNF- alpha in the cerebrospinal fluid of A and B groups were significantly higher than those in the control group, and the difference was statistically significant. The difference was not statistically significant (P0.05), but the level of IL-1 beta and TNF- alpha in the cerebrospinal fluid of C group was slightly lower than that of the control group (P0.05). Compared with the C group, the level of IL-1 beta, IL-6 and TNF- alpha in the cerebrospinal fluid of the A group and B group was significantly higher than that in the B group, and the difference was statistically significant (P0.05), compared with the group of cerebrospinal fluid, the level of the cerebrospinal fluid in the cerebrospinal fluid was significantly higher. The difference has statistical significance (P0.05), suggesting that the levels of IL-1 beta, IL-6 and TNF- alpha in cerebrospinal fluid in acute phase of viral encephalitis are related to acute epileptic seizures. Conclusion: the levels of IL-1 beta, IL-6 and TNF- a in the serum of 1. viral encephalitis patients with acute phase epilepsy are significantly higher than IL-1 beta in serum of patients with acute toxic encephalitis without epileptic seizures, IL The level of -6 and TNF- alpha suggests that the level of IL-1 beta, IL-6 and TNF- alpha in serum in acute phase of viral encephalitis is higher than that in the cerebrospinal fluid of patients with acute phase of.2. viral encephalitis. The level of IL-1 beta, IL-6 and TNF- is significantly higher than that of IL-1 beta, IL-6 and TNF- alpha in the cerebrospinal fluid of patients with acute viral encephalitis without epileptic seizures. Epileptic seizures in acute phase of toxic encephalitis are related to elevated levels of IL-1, IL-6 and TNF- alpha in CSF.
