自拟逐饮活血汤辅助治疗结核性胸膜炎的疗效及对炎症因子与肺功能的影响

【摘要】：目的观察自拟逐饮活血汤辅助治疗结核性胸膜炎的临床疗效及对患者胸腔积液炎症因子与肺功能的影响。方法将75例结核性胸膜炎患者随机分为2组,对照组38例给予胸腔穿刺抽液术联合抗结核药物治疗,观察组37例在对照组治疗基础上加用逐饮活血汤治疗,2组均以2周为1个疗程,共治疗3个疗程。观察2组治疗前后中医证候评分、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC变化情况,检测2组治疗前后胸腔积液中肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)水平,统计2组治疗后临床疗效及治疗期间不良反应发生情况。结果治疗后2组中医证候评分均显著降低(P均0.05),FVC、FEV1及FEV1/FVC显著提高(P均0.05),且观察组改善情况均明显优于对照组(P均0.05);治疗后2组TNF-α、CRP水平均显著降低(P均0.05),且观察组显著低于对照组(P均0.05);观察组治疗总有效率显著高于对照组(P0.05),不良反应发生率显著低于对照组(P0.05)。结论自拟逐饮活血汤可显著改善结核性胸膜炎患者的中医证候,提高肺功能,减轻炎症反应,疗效好,且不良反应更少,值得临床推广应用。
[Abstract]:Objective to observe the clinical effect of Zhuyin Huoxue decoction in treating tuberculous pleurisy and its influence on inflammatory factors and pulmonary function in pleural effusion. Methods Seventy-five patients with tuberculous pleurisy were randomly divided into two groups. 38 patients in the control group were treated with thoracic puncture and drainage combined with antituberculous drugs. In the observation group, 37 cases were treated with Zhuyin Huoxue decoction on the basis of the control group. All the 37 cases in the observation group were treated with 2 weeks as a course of treatment, with a total of 3 courses of treatment. The scores of TCM syndromes, forced vital capacity (FVC), forced expiratory volume (FEV1) and FEV1 / FVC were observed before and after treatment in both groups. The levels of tumor necrosis factor- 伪 (TNF- 伪) and C-reactive protein (CRP) in pleural effusion were measured before and after treatment. The clinical efficacy and adverse reactions during the treatment were analyzed in the two groups. Results after treatment, the scores of TCM syndromes in the two groups were significantly decreased (P 0.05), the FVCU FEV1 and FEV1 / FVC were significantly increased (P 0.05), and the improvement in the observation group was significantly better than that in the control group (P 0.05), and the level of TNF- 伪 -CRP in the two groups decreased significantly after treatment (all P 0.05), and the observation group showed significant improvement. The total effective rate of treatment in the observation group was significantly higher than that in the control group (P0.05), and the incidence of adverse reactions was significantly lower than that in the control group (P0.05). Conclusion Zhuyin Huoxue decoction can significantly improve the syndrome of tuberculous pleurisy, improve lung function, alleviate inflammatory reaction, have good curative effect, and have fewer adverse reactions, so it is worthy of clinical application.
