T-SPOT. TB试验对不同BMI组疑似肺结核患者的诊断价值分析

【摘要】:[目的]探讨结核感染T细胞斑点试验(T-SPOT.TB)对不同BMI组疑似肺结核患者的诊断价值,分析BMI与血常规淋巴细胞计数、血常规单核细胞数及血结核感染T细胞斑点试验中致敏的结核杆菌特异性T淋巴细胞数(即斑点数)是否有相关性。[方法]1.回顾性分析2015年1月1日至2016年12月31日昆明医科大学第一附属医院呼吸内科收治并行T-SPOT.TB检测的疑似肺结核病例共240例。经完善痰和支气管肺泡液抗酸杆菌涂片及和结核分枝杆菌培养、组织病理学检查等,最终诊断为肺结核患者138例,非肺结核患者102例。按照BMI指数分为A组(即消瘦组BMI18.6kg/m2)、B 组(即正常组 18.6≤BMI≤22.9kg/mm2)、C 组(即超重和肥胖组BMI≥23kg/m2)。2.记录入选患者出院诊断、年龄、性别、身高、体重、体重指数,并记录结核感染T细胞检测结果、入院后第一次血常规的淋巴细胞计数和单核细胞数,将T-SPOT.TB结果与诊断结果进行比较,分析T-SPOT.TB的诊断效能。性别计数资料用卡方检验,其它计数资料采用率及构成比表示,组间比较采用Youden指数和诊断准确率,P0.05表示有统计学意义。基本描述计量资料为正态分布或者近似正态分布,采用均数±标准差表示,组内比较采用配对t检验,各组间两两对比采用方差分析中多个样本均数的两两比较q检验。[结果]本研究共纳入研究对象240例,男性153名,女性87名,性别比例为1.76,年龄最小为18岁,最大为87岁,平均年龄为49.41 ±18.81岁。其中A组45人,B组162人,C组33人。三组间年龄性别差异无统计学意义。T-SPOT.TB试验对B组的敏感性及特异性均较高,而对A组和C组敏感性及特异性均较差。血常规T淋巴细胞数分析B组C组A组,组间比较仅A组和B组差异有统计学意义。血常规单核细胞数分析B组C组A组,A组和B组差异有统计学意义,A组和C组差异有统计学意义,C组和B组差异无统计学意义。三组患者致敏性T淋巴细胞数组内比较A孔斑点数和B孔斑点数无统计学差异,三组中A孔斑点数有统计学差异,B组大于A组和C组。三组中B孔斑点数差异有统计学意义,B组大于A组和C组。[结论]1.消瘦、超重及肥胖均能降低T-SPOT.TB试验敏感性、特异性、阳性预测值及阴性预测值,从而导致T-SPOT.TB试验对肺结核的诊断准确率下降;2.消瘦、超重及肥胖患者,机体免疫功能紊乱,导致结核感染后致敏性T淋巴细胞减少,γ-干扰素释放减少,T-SPOT.TB结果A孔和B孔斑点数减少;3.消瘦患者T淋巴细胞下调更明显,导致T-SPOT.TB试验对肺结核诊断价值下降;4.单核细胞高的患者,T-SPOT.TB结果A孔和B孔斑点数也较高。
