HBeAg阳性慢性乙肝患者HBsAg定量与HBeAg、HBV DNA、基因型的相关性

【摘要】：目的探讨HBe Ag阳性的慢性乙型肝炎患者HBs Ag定量与HBe Ag、HBV DNA、HBV基因型的关系。方法收集2014年1~12月于烟台市传染病医院首诊的154例HBe Ag阳性慢性乙型肝炎患者的资料,比较不同基因型患者HBs Ag定量、HBe Ag定量及HBV DNA水平,并采用Sperman相关系数评价HBs Ag定量与HBe Ag水平和HBVDNA定量的相关性。结果154例患者基因型分布为:B型48例,C型104例,非B非C型2例。C基因型HBe Ag阳性CHB患者的HBs Ag水平[4.0(2.6,5.5)log10IU/ml]和HBV DNA水平[6.8(4.3,8.5)log10 copies/ml]高于B基因型患者[4.0(2.6,5.0)log10IU/ml,6.5(3.7,7.8)log10copies/ml],且差异有统计学意义(均P0.05),HBe Ag水平二者差异无统计学意义。采用sperman相关分析比较HBs Ag定量、HBe Ag定量及HBV DNA定量的关联,结果HBs Ag水平与HBe Ag水平呈负相关,相关系数r=-0.42,P=0.005;HBs Ag水平与HBV DNA水平呈负相关,r=-0.43,P=0.03。结论 HBs Ag定量与HBe Ag、HBV DNA、乙肝基因分型存在一定程度的关联,建议同时检测上述指标以指导临床诊疗。
[Abstract]:Objective to investigate the relationship between HBs Ag quantification and HBe Ag,HBV DNA,HBV genotype in patients with HBe Ag positive chronic hepatitis B. Methods the data of 154 patients with HBe Ag positive chronic hepatitis B who were first diagnosed in Yantai Infectious Disease Hospital from January to December 2014 were collected and the quantitative, HBe Ag and HBV DNA levels of HBs Ag were compared among different genotypes. Sperman correlation coefficient was used to evaluate the correlation between HBs Ag quantification, HBe Ag level and HBVDNA quantification. Results the genotypes of 154 patients were as follows: type B 48, C 104. The levels of HBs Ag [4.0 (2.6 ~ 5.5) log10IU/ml] and HBV DNA [6.8 (4.3 ~ 8.5) log10 copies/ml] in patients with HBe Ag positive CHB were significantly higher than those in patients with genotype B [4.0 (2.6 ~ 5.0) log10IU/ml,6.5 (3.77.8) log10copies/ml], and there was no significant difference between them (P0.05). Sperman correlation analysis was used to compare HBs Ag quantitative, HBe Ag quantitative and HBV DNA quantitative correlation. The results showed that there was a negative correlation between HBs Ag level and HBe Ag level, and a negative correlation between HBs Ag level and HBV DNA level, and a negative correlation between HBs Ag level and HBV DNA level. Conclusion the quantitative analysis of HBs Ag is associated with the genotyping of HBe Ag,HBV DNA, hepatitis B to some extent. It is suggested that the above indexes should be detected simultaneously to guide clinical diagnosis and treatment.
