2013年开江县肺吸虫病流行病学调查

【摘要】:目的摸清开江县易感人群和中间宿主肺吸虫感染情况,为防制提供科学依据。方法 2013年9-10月,在开江县历年有肺吸虫病病例的沙坝乡抽取小学生和村民,采血检测肺吸虫抗体;捕捉钉螺和螃蟹分别检测肺吸虫尾蚴及囊蚴。结果检测440人,血清肺吸虫抗体阳性的28人,阳性率为6.36%。抗体阳性率,男性为7.83%,女性为4.76%(P0.05);各个年龄组在0%~10.17%(P0.05)。检测拟钉螺196只,检出肺吸虫尾蚴的29只,感染率为14.80%;查出尾蚴共38条,钉螺感染度为0.19条。检查螃蟹209只,检出肺吸虫囊蚴的31只,感染率为14.83%,检出囊蚴共71个,螃蟹感染度为0.34个。调查的440人中,喝生水的占83.86%,玩螃蟹的占68.18%,吃螃蟹的占56.82%。生吃螃蟹者所占比例,为全部调查对象的10.91%,调查对象中肺吸虫血清抗体阳性者所占比例,生吃螃蟹者为16.67%和半生吃螃蟹者的10.22%,显著高于熟吃和不吃螃蟹者的0.49%(P0.01);喝生水者为7.32%与不喝生水者的1.41%无统计学意义(P0.05);玩螃蟹者为8.33%高于不玩螃蟹者为2.14%(P0.05)。结论开江县为肺吸虫病低流行区,肺吸虫感染主要是生吃和半生吃螃蟹所致。
[Abstract]:Objective to investigate the infection of paragonimiasis in susceptible population and intermediate host in Kaijiang County, and to provide scientific basis for prevention and control. Methods from September to October, 2013, primary school students and villagers were selected from Shaba Township with paragonimiasis cases in Kaijiang County, and blood samples were collected to detect paragonimiasis antibodies, and snails and crabs were captured to detect cercariae and cysticercariae, respectively. Results 440 patients were detected, 28 of them were positive for paragonimiasis antibody, the positive rate was 6.36. The positive rate of antibody was 7.83% in male and 4.76% in female (P0.05), and 10.17% in each age group (P0.05). 196 Oncomelania hupensis were detected and 29 cercariae of paragonimiasis were detected, the infection rate was 14.80 and 38 cercariae were detected, the infection degree of Oncomelania hupensis was 0.19. Of the 209 crabs, 31 were found to have paragonimiasis, the infection rate was 14.83, the cysticercaria was 71, and the infection degree of crab was 0.34. Of the 440 people surveyed, 83.86 drank raw water, 68.18 played with crabs and 56.82 ate crabs. The proportion of those who ate crab raw was 10.91.The proportion of those who had positive antibody to paragonimiasis serum was 16.67% and 10.22% of those who ate crab raw and those who ate crab half-raw were 16.67%, 10.22% and 10.22%, respectively. Significantly higher than cooked and not eating crab 0.49% (P0.01); There was no significant difference between those who drank raw water (7.32%) and those who did not drink raw water (1.41%) (P0.05), while those who played crabs were 8.33% higher than those who did not play crabs 2.14% (P0.05). Conclusion Kaijiang County is a low prevalence area of paragonimiasis. The infection of paragonimiasis is mainly caused by raw and semi-raw crabs.
