犬瘟热病例生化指标的变化及意义

【摘要】:目的犬瘟热病毒(Canine distemper virus,CDV)长期以来因其强传染力,高致死力,而被誉为犬类的第1号致死性疾病。尽管随着宠物医疗药物、技术及设备的进步,CDV的治愈率虽有所提高,但治愈率仍很低下,主要体现在CDV对机体的损害是多系统多器官性的,且治疗方案多为对症治疗。迄今为止,CDV发病时期器官功能学研究较少。机体各系统的功能变化不清,可能是导致治愈率低的主要原因之一。本课题对CDV患犬在治疗中各种脏器生化的指标变化进行了系统的研究,旨在弄清CDV患犬生化指标的变化及意义,阐明其相关机制,为CDV临床治疗提供指导,进而提高CDV的治愈率。方法(1)试验设计:本研究以洛阳市某宠物医院前来就诊的CDV患犬为实验组,以体检且健康的犬为对照组,进行血清生化指标的连续的系统的监测。分析其生化单项指标的总体变异率及升降关系,生化指标功能的变化规律,以及CDV痊愈犬与死亡犬生化指标的对比,来找出CDV与各生化指标的相关性。(2)数据处理:?单项指标总体变异率:在检测报告单中,指标超出参考范围上限值或低于下限值,计为变异1次,对所有样本每个生化指标做累加处理,即得变异样本总数。然后,将变异样本总数除以样本总数,即为变异样本率;?生化指标功能变化率:将生化检测报告数据,按其功能性归为肝功,肾功,心功,胰功,血清钙,以及蛋白六类。所测指标超出参考范围上限值或低于下限值,计为变异1次,对所有样本每项指标做累加处理,即为变异样本总数。然后,将变异样本总数除以样本总数,即为变异样本率;?CDV痊愈犬与死亡犬生化指标的对比:实验分痊愈,死亡,对照三组。将每组每项生化指标值汇总后取平均值,用统计软件进行组间差异显著性分析。结果(1)CDV病例生化指标的单项变异率的临界值约为30%。变异率30%的有ALP、CK、r-GT、TBIL、Ca、ALB、AST、P、TP、CREA,这些生化指标有显著统计学意义(P0.05);变异率30%的指标有TC、LIP、GLB、ALT、GLU、UREA、TG、AMY,这些生化指标对犬瘟病例治疗及预后的影响不大,无统计学意义。(2)CDV病例脏器生化功能变异率的临界值是40%;若变异率≥40%,说明脏器功能有显著变化,显著性变化的脏器(指标)有心功、血清钙、肝功、蛋白质、肾功。若脏器功能变异率40%,说明脏器功能变化不显著。无显著变化脏器(指标)有胰腺功能、血糖血脂。(3)对痊愈组与死亡组进行对比结果显示,有显著性差异的是胰腺功能降低、血清钙浓度下降及蛋白质缺乏,这可能是导致CDV患犬最终死亡的主要原因。结论结果表明,CDV患犬在治疗期间,应及时监测ALP、CK、r-GT、TBIL、Ca、ALB、AST、P、TP、CREA等生化指标,以及胰腺功能、血清钙及蛋白质指标的变化,根据检测指标,随时调整临床用药,可能会提高CDV的治疗率,降低死亡率。
[Abstract]:Objective canine distemper virus (canine distemper virus) has long been regarded as the first fatal disease of dogs because of its strong infectivity and high lethal power. Although the cure rate of CDV has been improved with the progress of pet medicine, technology and equipment, the cure rate is still very low, which is mainly reflected in the fact that the damage of CDV to the body is multi-system and multi-organ, and most of the treatment schemes are symptomatic. So far, there are few studies on organ function during the onset of CDV. Unclear functional changes in the system may be one of the main causes of low cure rate. In this paper, the changes of biochemical indexes of various organs in dogs with CDV were studied systematically. The purpose of this study was to clarify the changes and significance of biochemical indexes in dogs with CDV, to clarify the related mechanism, and to provide guidance for the clinical treatment of CDV. Then the cure rate of CDV was improved. Methods 1) the experimental design: the CDV dogs in a pet hospital in Luoyang were used as the experimental group and the healthy dogs as the control group. The serum biochemical indexes were monitored systematically and continuously. In order to find out the correlation between CDV and each biochemical index, the relationship between the total variation rate and the rise and fall of the single biochemical index, the change rule of the biochemical index function, and the comparison between the biochemical index of CDV cured dog and dead dog were analyzed to find out the correlation between CDV and each biochemical index. The total variation rate of single index: in the test report sheet, the index exceeds the upper limit value of reference range or below the lower limit value, it is counted as variation one time, for each biochemical index of all samples to do cumulative processing, that is, the total number of variation samples. Then, divide the total number of samples by the total number of samples, that is, the sample rate of variation? The change rate of biochemical index function: according to its function, the data reported by biochemical test were classified as liver function, kidney function, heart function, pancreatic function, serum calcium and protein. If the measured index exceeds the upper limit of the reference range or is below the lower limit, it is counted as one variation, and the total number of variation samples is added to each index of all samples. Then, the total number of variation samples was divided by the total number of samples, that is, the rate of variation sample was compared with the biochemical indexes of CDV cured dogs and dead dogs: the experiment was divided into three groups: recovery, death and control. The average value of each biochemical index in each group was collected and the statistical software was used to analyze the significance of the difference between groups. Results the critical value of single variation rate of CDV was about 30%. The rate of variation was 30% in ALP CK r-GTT TBILA, and the biochemical indexes were statistically significant (P 0.05), and the variation rate was 30% in TCU LIPGLBALBALL GLUEAEAA TGAMY.These biochemical indexes had little effect on the treatment and prognosis of canine plague. The results showed that the biochemistry parameters had no significant effect on the treatment and prognosis of canine plague patients, and the variation rate of 30% was higher than that of the control group (P < 0.05). The results showed that there was no significant difference in the treatment and prognosis of canine plague patients. The critical value of organ biochemical function variation rate in CDV patients with no statistical significance was 40. If the variation rate was greater than 40, it indicated that the organ function had significant changes, and the organs (indexes) with significant changes had heart function, serum calcium, liver function, protein and kidney function. If the variation rate of organ function is 40%, the change of organ function is not significant. No significant changes were found in organs (indicators) with pancreatic function, blood glucose, blood lipid and blood lipid. The results of comparison between the cured group and the dead group showed that the pancreatic function was decreased, the serum calcium concentration was decreased and the protein deficiency was found in the cured group. This may be the leading cause of death in dogs with CDV. Conclusion during the treatment period, the changes of biochemical indexes, pancreatic function, serum calcium and protein index, and so on, should be monitored in dogs with CDV. The treatment rate of CDV may be improved by adjusting the clinical medication at any time. Reduce mortality.
