犬高血糖症与血钙、血磷和血镁代谢的相互关系

【摘要】:高血糖症是由于血糖升高而引起的一种代谢综合征。临床上,当犬的血糖浓度高于7.3 mmol/L(130 mg/dL)时,就认为存在犬高血糖症。高血糖是糖尿病的重要指标,也是诱发糖尿病以及肝脏、肾脏和心血管疾病的重要因素。近年来,机体内元素与代谢综合征发生发展关系的研究层出不穷,人医上已经有很多关于糖尿病患者血液中元素代谢失衡的研究,但是兽医临床上对这方面的研究还很缺乏。本论文旨在探讨高血糖患犬是否存在血钙、血磷或血镁的代谢异常,以期为临床高血糖患犬的管理及其相关继发症的防治提供新的研究思路。论文收集了台湾中兴大学兽医教学医院2015年1月至2016年10月近2年患犬的门诊病例资料。根据病史、临床检查、实验室检查和特殊检查等病例资料进行筛选,排除患有恶性肿瘤、骨质疏松、严重胃肠道疾病、酮症酸中毒和严重肾脏疾病等病例。最终将血糖正常(70~120 mg/dL)且各项检查均无明显异常的110例犬作为对照组。将血糖浓度高于130 mg/dL,临床上未诊断出糖尿病的119例患犬作为非糖尿病高血糖组。将血糖浓度高于200 mg/dL,临床上确诊为糖尿病的22例患犬作为糖尿病高血糖组。通过统计学分析方法比较了三组不同血糖水平犬的血钙、血磷和血镁水平之间的差异,并对血糖与血钙、血磷和血镁水平之间的相关性进行了分析。结果显示:非糖尿病高血糖组的血钙显著低于对照组(p0.05),而血镁和血磷在两组间无显著性差异(p0.05);糖尿病高血糖组的血钙和血镁均显著低于对照组(p0.05),而血磷在两组间无显著性差异(p0.05)。血糖与血钙、血镁呈显著的负相关性(p0.05,r=-0.320,-0.152),与血磷呈显著的正相关性(p0.05,r=0.124),其中血糖与血钙的相关性最显著。结果表明,高血糖患犬存在血钙、血镁和血磷的代谢异常。作者认为在高血糖患犬中,通过密切监测血钙、血镁和血磷的浓度变化,适当地给患犬补充钙、镁制剂和维生素D可能有利于改善高血糖患犬的预后,对高血糖患犬给予适量的排磷药物可能有利于延缓高血糖对肾脏的损伤。
[Abstract]:Hyperglycemia is a metabolic syndrome caused by elevated blood sugar. Clinically, canine hyperglycemia is thought to exist when blood glucose levels are higher than 7.3 mmol/L(130 mg / dL. Hyperglycemia is an important indicator of diabetes and an important factor in inducing diabetes as well as liver, kidney and cardiovascular disease. In recent years, there have been many studies on the relationship between elements and metabolic syndrome in the body. There have been many studies on the imbalance of element metabolism in the blood of patients with diabetes mellitus. However, there is still a lack of research on this aspect in veterinary clinic. The purpose of this study was to investigate the metabolic abnormalities of serum calcium, phosphorus or magnesium in dogs with hyperglycemia, in order to provide a new approach for the management of hyperglycemic dogs and the prevention and treatment of related secondary disorders. The paper collected the outpatient cases of dogs from January 2015 to October 2016 in the Veterinary Teaching Hospital of ZTE University in Taiwan. Cases of malignant tumor, osteoporosis, severe gastrointestinal diseases, ketoacidosis and severe renal diseases were excluded according to the history, clinical examination, laboratory examination and special examination. In the end, 110 dogs with normal blood glucose (70 mg / dL) and no obvious abnormal examination were taken as the control group. A total of 119 dogs with no diabetes were treated as non-diabetic hyperglycemia group with blood glucose concentration higher than 130 mg / dL. Twenty-two dogs with diabetes were selected as diabetic hyperglycemia group, whose blood glucose level was higher than 200 mg / dL. The differences of serum calcium, phosphorus and magnesium levels among three groups of dogs with different blood glucose levels were compared by statistical analysis, and the correlation between blood glucose and serum calcium, phosphorus and magnesium levels was analyzed. The results showed that the level of serum calcium in the non-diabetic hyperglycemia group was significantly lower than that in the control group (P 0.05), but there was no significant difference in serum magnesium and phosphorus between the two groups, while the levels of serum calcium and magnesium in the diabetic hyperglycemia group were significantly lower than those in the control group (P 0.05), but there was no significant difference in serum phosphorus between the two groups. There was a significant negative correlation between blood glucose and blood calcium, and a significant negative correlation between blood magnesium and blood glucose, and a significant positive correlation between blood glucose and serum phosphorus, among which the correlation between blood glucose and serum calcium was the most significant. The results showed that there were abnormal metabolism of serum calcium, magnesium and phosphorus in dogs with hyperglycemia. The authors suggest that the proper supplement of calcium, magnesium and vitamin D may be helpful to improve the prognosis of hyperglycemic dogs by closely monitoring the changes of serum calcium, magnesium and phosphorus concentrations. Proper dose of phosphorus excretion in hyperglycemic dogs may be beneficial to delay renal damage caused by hyperglycemia.
