儿童紫癜性肾炎热伤肾络证代谢组学研究

【摘要】:目的:本研究试图运用代谢组学研究思路和技术验证前期研究结果对于儿童紫癜性肾炎中医分型的合理性;明确儿童紫癜性肾炎尿液代谢物的变化特点,探索疾病的生物标志物;明确儿童紫癜性肾炎热伤肾络证尿液代谢物的变化特点,探索证候的生物标志物;从代谢组学水平探讨儿童紫癜性肾炎中医证型的物质基础及诊断标准。方法:采集符合纳入标准的儿童紫癜性肾炎热伤肾络证及非热伤肾络证患儿各9例,及健康体检者22例的空腹晨尿中段尿,-80℃冰箱存储,样本采集齐后利用超高液相色谱-离子肼质谱系统进行谱图扫描,取得各样品的代谢指纹谱,经谱图预处理,得出具有保留时间、质荷比、峰面积的三维数据,利用多元统计分析方法中的主成分分析考察样本分布总体情况,进一步使用偏最小二乘法及经正交校正的偏最小二乘法对各组的代谢指纹谱进行比较,根据载荷图,以及主成分贡献值及T检验,得出区别各组间的重要化合物,这些化合物可能是疾病和证型的“生物标记物群”结果:(1)儿童紫癜性肾炎和正常对照之间可以明显区分。(2)儿童紫癜性肾炎热伤肾络证和正常对照组可以明显区分。(3)儿童紫癜性肾炎非热伤肾络证组样本分布较散乱。(4)根据数据库检索,区别儿童紫癜性肾炎的化合物可能是羟基络醇4-硫酸酯、精氨酰丝氨酸、雌酮、色氨酰蛋氨酸、维生素K2、钝叶素葡萄糖苷、S-氨甲基二氢硫、苯丙精氨酸、L-蛋氨酸、同型半胱氨酸硫内酯、胆甾-3,7,12,25四醇-3-葡糖苷酸、磷脂酰胆碱、乙酰乙酸、N2,N2-二甲基鸟苷;区别儿童紫癜性肾炎热伤肾络证的化合物可能是丙酸、醛固酮、均聚L-精氨酸、维生素K2、钝叶素葡萄糖苷、S-氨甲基二氢硫、苯丙精氨酸、L-蛋氨酸、同型半胱氨酸硫内酯、羟基络醇4-硫酸酯、精氨酰丝氨、雌酮、色氨酰蛋氨。结论:(1)代谢组学的研究方法可以成功区分儿童紫癜性肾炎和正常对照组,可能的差异化合物有羟基络醇4-硫酸酯、精氨酰丝氨酸、雌酮、色氨酰蛋氨酸、维生素K2、钝叶素葡萄糖苷、S-氨甲基二氢硫、苯丙精氨酸、L-蛋氨酸、同型半胱氨酸硫内酯、胆甾-3,7,12,25四醇-3-葡糖苷酸、磷脂酰胆碱、乙酰乙酸、N2,N2-二甲基鸟苷。(2)代谢组学的研究方法可以成功区分儿童紫癜性肾炎热伤肾络证和正常对照组,可能的差异性化合物是丙酸、醛固酮、均聚L-精氨酸、维生素K2、钝叶素葡萄糖苷、S-氨甲基二氢硫、苯丙精氨酸、L-蛋氨酸、同型半胱氨酸硫内酯、羟基络醇4-硫酸酯、精氨酰丝氨酸、雌酮、色氨酰蛋氨酸。(3)儿童紫癜性肾炎热伤肾络证其代谢表征的确发生了变化,前期儿童紫癜性肾炎中医证型的研究结果中对于热伤肾络证证型的划分是有物质基础和科学性的。
[Abstract]:Objective: this study attempts to verify the rationality of the results of previous studies on TCM classification of Henoch-Schonlein purpura nephritis (HSPN) in children by means of metabonomics research ideas and techniques. The changes of urine metabolites in children with Henoch-Schonlein purpura nephritis (HSPN) and the biomarkers of the disease were investigated, and the changes of urinary metabolites in children with Henoch-Schonlein purpura nephritis (HSPN) were determined. To discuss the material basis and diagnostic criteria of TCM syndromes of Henoch-Schonlein purpura nephritis in children from the level of metabonomics.
