在不同切面中应用超声弹性成像面积比值法对BI-RADS 4级乳腺肿块良恶性判断的价值

【摘要】:目的:探讨在不同切面中应用超声弹性成像面积比值法对BI-RADS 4级乳腺肿块良恶性判断的价值。方法:对97例患者共116个BI-RADS 4级乳腺肿块分别进行肿块横径最大切面(A切面)、纵横比最大切面(B切面)、感兴趣部位最大面积切面(C切面)弹性成像检查,对弹性成像图像进行面积比值法分析。以病理诊断为金标准,绘制受试者工作特征曲线,判断三个不同切面对BI-RADS 4级乳腺肿块的诊断价值。结果:A、B、C三个切面BI-RADS 4级乳腺肿块良、恶性组间面积比值差异均具有统计学意义。三个切面所构建ROC曲线间比较差异无统计学意义。A切面面积比值以1.33为临界点判断肿块良恶性的敏感性、特异性、准确性分别为75.8%、95.2%、89.7%;B切面面积比值以1.25为临界点判断肿块良恶性的敏感性、特异性、准确性分别为84.8%、85.5%、85.3%;C切面面积比值以1.55为临界点判断肿块良恶性的敏感性、特异性、准确性分别为81.8%、95.2%、91.4%。结论:A、B、C三个不同切面超声弹性成像面积比值均有助于提高BI-RADS 4级乳腺肿块良恶性鉴别诊断。
[Abstract]:Objective: to evaluate the value of ultrasound elastic imaging area ratio method in the diagnosis of benign and malignant breast masses in BI-RADS grade 4 in different sections. Methods: a total of 116 BI-RADS grade 4 breast masses in 97 patients were examined by elastic imaging with maximum transverse diameter (A section), maximum aspect of aspect ratio (B section) and maximum area of interest (C section). The area ratio method was used to analyze the elastic imaging images. According to the gold standard of pathological diagnosis, the operating characteristic curve of the subjects was drawn to judge the diagnostic value of three different cuts to the BI-RADS grade 4 breast masses. Results there were significant differences in the area ratio among the three BI-RADS grade BI-RADS grade 4 breast masses. There was no significant difference between the ROC curves constructed by the three sections. The ratio of the area of section to the section area was 1.33 as the critical point to judge the sensitivity and specificity of benign and malignant tumors. The sensitivity, specificity and accuracy of 84.8and 85.3C section area ratio of 85.3C to 1.55 were used to judge the sensitivity, specificity and accuracy of the tumor, respectively. The sensitivity, specificity and accuracy were 81.8% and 95.21.4%, respectively, according to the critical point of 1.55. Conclusion the area ratio of ultrasound elastic imaging on three different sections is helpful to improve the differential diagnosis of benign and malignant breast masses in grade 4 of BI-RADS.
