【摘要】:1目的近年来妊娠期高血压疾病(HDCP)患者尤其是子痫前期孕妇增多,成为我国孕产妇和围产儿发病及死亡的主要原因之一。为了寻找能灵敏的反映妊娠期高血压疾病病理变化的有效指标,研究血管内皮生长因子(VEGF)和5-羟色胺(5-HT)在妊娠期高血压疾病患者与正常妊娠妇女外周血中的表达情况,探讨其与妊娠高血压疾病发病的关系,为早期预防HDCP的发生及临床诊断和治疗提供理论依据。方法1.选取滕州市中心人民医院2012年11月-2013年10月在产科住院的妊娠期高血压疾病患者50例为实验组(年龄23-35岁),其中妊娠期高血压组30例,子痫前期组20例。选取同期在我院住院的正常妊娠妇女52例为对照组(年龄24-37岁),选择的所有病例均排除内外科合并症及其他病理产科疾病如:前置胎盘、胎盘早剥、胎膜早破等。2.应用双抗体酶联免疫吸附法(ELISA)检测对照组与实验组外周血中VEGF和应用高效液相色谱-电化学检测(HPLC-ECD)法检测对照组与实验组外周血中5-HT的表达情况。3.采用SPSS统计软件进行方差分析,两组间均数比较采用t检验,血管内皮生长因子(VEGF)与5-羟色胺(5-HT)水平的相关性采用Pearson相关分析,以a=0.05为检验水准,P0.05为差异有统计学意义。结果1.妊娠期高血压疾病孕妇血清VEGF水平明显低于正常孕妇,P0.05差异有统计学意义。2.妊娠期高血压组血清VEGF水平较子痫前期组高,P0.05差异有统计学意义。3.随妊娠期高血压疾病血压的升高VEGF水平呈逐渐下降趋势,P0.05差异有统计学意义。4.妊娠期高血压疾病孕妇血浆中5-HT较正常孕妇明显升高,P0.05差异有统计学意义。5.子痫前期组血清中5-HT水平较妊娠期高血压组升高,P0.05差异有统计学意义。6.妊娠期高血压疾病组血VEGF与5-HT水平呈负相关,而在正常妊娠组中两者无明显相关性。结论1.VEGF和5-HT与妊娠期高血压疾病特别是子痫前期的发生及病情进展关系密切。2.VEGF在维持正常妊娠和胎儿生长发育过程中起积极作用。3.5-HT与妊娠期高血压疾病有一定的相关性。4.妊娠后母体血液中VEGF水平显著下降可能是妊娠期高血压疾病发病机理中的一个重要因素。5.VEGF和5-HT的水平变化反映妊娠期高血压疾病病情的动态变化,可作为妊娠期高血压疾病早期预测、临床诊断和治疗的参考依据。
[Abstract]:Objective in recent years, the number of patients with hypertension complicating pregnancy (HDCPD), especially preeclampsia, has increased. It has become one of the main causes of morbidity and mortality of pregnant women and perinatal infants in China. In order to find an effective index to reflect the pathological changes of hypertensive disorder complicating pregnancy. To study the expression of vascular endothelial growth factor (VEGF) and serotonin (5-HT) in peripheral blood of hypertensive disorder complicating pregnancy and normal pregnant women. Objective: to investigate the relationship between pregnancy hypertension and the incidence of pregnancy hypertension. To provide theoretical basis for early prevention, clinical diagnosis and treatment of HDCP. 1. Select the pregnant women hospitalized in obstetrics department of Tengzhou central people's hospital from November 2012 to October 2013. The experimental group consisted of 50 patients with compression disease (. Ages 23 to 35 years). There were 30 cases of hypertensive complicating pregnancy group and 20 cases of preeclampsia group. 52 normal pregnant women in our hospital were selected as control group (aged 24-37 years). All the selected cases were excluded from internal and surgical complications and other pathological obstetrical diseases such as placenta previa and placental abruption. Detection of VEGF in peripheral blood of control group and experimental group by double antibody enzyme-linked immunosorbent assay (Elisa) and high performance liquid chromatography-electrochemical detection (HPLC-ECD). Methods to detect the expression of 5-HT in peripheral blood of control group and experimental group by using SPSS software. The correlation between vascular endothelial growth factor (VEGF) and serotonin (5-HT) was analyzed by Pearson correlation analysis. Results 1. The serum VEGF level of pregnant women with hypertension complicating diseases was significantly lower than that of normal pregnant women. 2. The serum VEGF level of hypertensive complicating pregnancy group was higher than that of preeclampsia group. P0.05 difference was statistically significant. 3. With the increase of blood pressure of gestational hypertension disorder, the level of VEGF decreased gradually. P0.05 difference was statistically significant. 4. Gestational hypertension complicating hypertension in maternal plasma 5-HT significantly higher than normal pregnant women. The serum 5-HT level in preeclampsia group was higher than that in gestational hypertension group. P0.05 the difference was statistically significant. 6. The serum VEGF and 5-HT levels were negatively correlated in hypertensive disorder complicating pregnancy group. Conclusions 1. VEGF and 5-HT are closely related to the occurrence and progression of hypertensive disorder complicating pregnancy, especially preeclampsia. 3.5-HT plays a positive role in pregnancy and fetal development. 3.5-HT has a certain correlation with hypertensive disorder complicating pregnancy .4.After pregnancy, the level of VEGF in maternal blood decreased significantly, which may be the reason of hypertension complicating pregnancy. An important factor in the pathogenesis of the disease. 5. The changes of VEGF and 5-HT levels reflect the dynamic changes of hypertensive disorder complicating pregnancy. It can be used as a reference for early prediction, clinical diagnosis and treatment of hypertensive disorder complicating pregnancy.
