【摘要】:目的:通过检测水通道蛋白9(AQP9)和凋亡蛋白Caspase7在正常羊水量孕产妇和羊水过少孕产妇胎盘及胎膜中的水平,探讨AQP9和Caspase7与羊水过少的关系及其二者的相关性。 方法:选择2013年6月至2013年11月在延安大学附属医院产科足月孕(37-41周)剖宫产分娩的羊水过少组孕产妇及正常组孕产妇各30例,两组孕产妇年龄、孕周、孕次等均无显著性差异,既往均无内外科合并症及其他妊娠合并症(如妊娠期高血压疾病、胎儿宫内生长受限、胎膜早破、胎儿畸形等),近期无药物服用史、感染史及母体脱水等因素。采用免疫组化法(SP法)分别检测30例正常羊水量和30例羊水过少孕产妇胎盘及胎膜组织中AQP9和Caspase7表达水平,对两种蛋白的表达强度行半定量分析,并确定其表达部位。 结果: 1. Caspase7、AQP9在两组孕产妇胎盘组织中的表达 Caspase7、AQP9蛋白在两组产妇的胎盘的细胞中均有阳性表达,,呈棕黄色,其阳性表达部位主要分布于胎盘合体滋养细胞的细胞膜和胞浆中。羊水过少组孕产妇胎盘滋养细胞中Caspase7的表达水平高于对照组(t=2.85),而AQP9蛋白的表达水平却低于对照组(t=2.32),差异均有统计学意义(P<0.05). 2. Caspase7、AQP9在两组孕产妇胎膜组织中的表达 Caspase7、AQP9蛋白在两组产妇的胎膜组织的细胞中均有阳性表达,呈棕黄色,其阳性表达部位主要分布于胎膜上皮细胞的细胞膜和胞浆中。羊水过少组孕产妇胎膜细胞中Caspase7的表达水平高于对照组(t=3.12),而AQP9蛋白的表达水平却低于对照组(t=4.16),差异均有统计学意义(P<0.05)。 3. Caspase7、AQP9在两组孕产妇胎盘胎膜组织中的表达关系 经Spearman等级相关分析后发现:正常组孕产妇胎盘滋养细胞及胎膜上皮细胞Caspase7与AQP9的表达系数分别为rs=0.12,rs=0.23,P均大于0.05,差异均无统计学意义;而在羊水过少组其值分别为rs= 0.63, rs= 0.78,P值均小于0.05,差异均有统计学意义。 结论: 1.两组孕产妇胎盘胎膜中Caspase7蛋白与AQP9蛋白的表达差异均有统计学差异。因此羊水过少孕妇胎盘胎膜中Caspase7蛋白的高表达与AQP9蛋白的低表达,提示AQP9和Caspase7在产妇胎盘及胎膜中的异常表达与羊水过少的发生有密切关系。 2.正常组孕产妇胎盘胎膜细胞中Caspase7与AQP9的表达均无相关性关系,而在羊水过少组胎盘胎膜细胞中Caspase7与AQP9的表达水平均呈负相关。由此我们推测在羊水过少组的孕妇胎盘胎膜细胞上Caspase7的表达增加可诱导细胞自身凋亡增加,使得胎盘胎膜上AQP9的表达下降,影响胎盘胎膜上AQP9的结构甚至功能,导致物质交换障碍,进一步影响体内的水代谢及其循环,从而降低羊水等物质在母体、胎儿及胎膜之间的交换,导致羊水过少。
[Abstract]:Objective: to detect the levels of aquaporin 9 (AQP9) and apoptotic protein Caspase7 in placenta and membranes of normal amniotic fluid pregnant women and oligohydramnios women. To investigate the relationship between AQP9 and Caspase7 and oligohydramnios. Methods: 30 pregnant women with oligohydramnios and 30 normal pregnant women were selected from June 2013 to November 2013. There was no significant difference between the two groups in age, gestational week and second trimester. There were no complications of internal and surgical diseases and other pregnancy complications (such as hypertensive disorder complicating pregnancy, fetal growth restriction, premature rupture of membranes). Fetal malformation, no recent history of drug use. History of infection and maternal dehydration. Immunohistochemical method (SP method). The expression of AQP9 and Caspase7 in placenta and membranes of 30 cases of normal amniotic fluid and 30 cases of oligohydramnios were detected respectively. The expression intensity of the two proteins was determined by semi-quantitative analysis. Results: 1. Expression of Caspase7 and AQP9 in placenta of pregnant and parturient women The expression of Caspase7 and AQP9 protein in placental cells of both groups was brown and yellow. The expression of Caspase7 in placental trophoblastic cells in oligohydramnios group was higher than that in control group (2.85). . The expression of AQP9 protein was significantly lower than that of control group (P < 0.05). 2. Expression of Caspase7 and AQP9 in fetal membrane of pregnant and parturient women The expression of Caspase7 and AQP9 protein was brown in the cells of fetal membrane tissue of the two groups. The expression of Caspase7 in fetal membranes of oligohydramnios group was higher than that of control group (3.12). The expression of AQP9 protein was significantly lower than that of control group (P < 0.05). 3. Expression of Caspase7 and AQP9 in placental membranes of pregnant and parturient women Spearman grade correlation analysis showed that the expression coefficients of Caspase7 and AQP9 in placental trophoblastic cells and fetal membrane epithelial cells of normal pregnant women were rs=0.12 respectively. There was no significant difference between 0.23 P and 0.05; In oligohydramnios group, the values were 0.63, 0.78, P < 0.05, respectively. The difference was statistically significant. Conclusion: 1. The expression of Caspase7 protein and AQP9 protein in placental membranes of pregnant women with oligohydramnios was significantly different between the two groups. Therefore, the high expression of Caspase7 protein in placental membranes of oligohydramnios pregnant women. And the low expression of AQP9 protein. The results suggest that the abnormal expression of AQP9 and Caspase7 in placenta and membranes is closely related to the occurrence of oligohydramnios. 2. There was no correlation between the expression of Caspase7 and AQP9 in placental membranes of normal pregnant women. However, the expression of Caspase7 and AQP9 in placental membranes of oligohydramnios was negatively correlated with that in oligohydramnios. Therefore, we speculate that Caspase7 is expressed on placental membranes of pregnant women with oligohydramnios. The increase of cell apoptosis can be induced by the increase of cell apoptosis. The expression of AQP9 on placental membranes is decreased, the structure and even function of AQP9 on placental membranes are affected, the substance exchange is obstructed, and the water metabolism and its circulation are further affected. Thus reducing the exchange of amniotic fluid and other substances between the maternal, fetal and fetal membranes, resulting in oligohydramnios.
