宫颈癌患者同步放化疗期间症状群的调查研究

【摘要】：目的调查宫颈癌患者同步放化疗期间症状群和症状分布。方法 2015年11月—2016年6月,采用便利抽样法,在徐州市3所三级医院——徐州医科大学附属医院、徐州市中心医院、徐州矿物集团总医院放疗中心抽取符合纳入标准的宫颈癌患者137例,均接受同步放化疗治疗。采用一对一访谈式调查,调查问卷包括人口学资料、临床资料和中文版记忆症状评估量表(MSAS-Ch)。结果本研究共发放问卷137份,回收有效问卷124份,有效回收率为90.5%。124例宫颈癌患者平均年龄(51.5±8.6)岁。宫颈癌患者同步放化疗期间症状发生率位于前5位的分别是乏力(97.6%,121/124)、食欲不振(87.9%,109/124)、口干(83.9%,104/124)、皮肤改变(81.5%,101/124)、恶心(80.6%,100/124)。乏力发生率与食欲不振发生率、食欲不振发生率与进食口味改变发生率、口干发生率与腹泻发生率、皮肤改变发生率与腹胀发生率、恶心发生率与进食口味改变发生率、恶心发生率与腹泻发生率、进食口味改变发生率与腹泻发生率均呈正相关(P0.05)。探索性因子分析法得出7个症状群:胃肠道症状群、能量不足症状群、围绝经期症状群、自我形象受损症状群、躯体相关症状群、神经系统相关症状群、心理症状群,总方差贡献率为56.985%,各症状群的方差贡献率为9.972%、9.117%、8.256%、8.214%、7.376%、7.033%、7.017%,Cronbach'sα系数分别是0.853、0.812、0.798、0.870、0.775、0.846、0.913。结论宫颈癌患者同步放化疗期间存在多种症状群,临床护理人员应积极加强症状管理,提高患者治疗期间生活质量。
[Abstract]:Objective to investigate the patients with cervical cancer radiotherapy and chemotherapy during the symptoms and symptoms distribution. Methods from November 2015 to June 2016, with the convenient sampling method, in Xuzhou city 3 level three hospital, Xuzhou Medical University Affiliated Hospital, Xuzhou Central Hospital, Xuzhou mineral group general hospital radiotherapy center from 137 cervical cancer patients met the inclusion criteria were all chemoradiation therapy. The occurrence of fatigue and loss of appetite, loss of appetite and eating taste change incidence rate, incidence of dry mouth and diarrhea, skin changes in incidence and incidence of nausea and abdominal distension, eating taste change rate, incidence of nausea with diarrhea, eating taste change rate and diarrhea rate were positively correlated (P0.05). Exploratory factor analysis obtained 7 symptoms: gastrointestinal symptoms, energy deficiency symptoms of perimenopausal symptoms, self image damage symptoms, somatic symptoms, group related neurological symptoms, psychological symptoms group, the total variance contribution rate was 56.985%, the variance contribution of each symptom group was 9.972%, 9.117%, 8.256%, 8.214%, 7.376%, 7.033%, 7.017%, the Cronbach's coefficient is 0.853,0.812,0.798,0.870,0.775,0.846,0.913. conclusion There are a variety of symptom groups during the synchronous radiotherapy and chemotherapy of cancer patients. The clinical nurses should actively strengthen the management of symptoms and improve the quality of life during the treatment.
