子痫前期患者血清及胎盘中TGF-β1、Endoglin表达及其相关性研究

【摘要】:目的:本实验通过测定子痫前期患者外周血中TGF-β1、s Eng的浓度及胎盘组织中TGF-β1、Endoglin的表达水平,对比分析子痫前期和正常孕产妇胎盘组织中TGF-β1、Endoglin的表达及其血清对应的TGF-β1、s Eng的含量,研究其相关性,探讨TGF-β1、Endoglin、s Eng在子痫前期病因及发病机制中的作用,进一步为以后临床上预测、诊疗方面提供新的理论依据。方法:随机选取2014年6月-2014年12月在我院妇产科住院的子痫前期患者60例(其中轻度子痫前期组30例,重度子痫前期组30例),子痫前期的诊断标准参照谢幸主编的第八版妇产科学。选取血压正常的晚期孕产妇30例作为对照组。三组患者在年龄、孕周、分娩方式及孕次之间均无显著性差异。采用双抗体夹心酶联免疫吸附法(ELISA)测定研究对象外周血清中TGF-β1、s Eng浓度。胎盘组织中TGF-β1、Endoglin以免疫组化SP法检测。将所得的数据用SPSS19.0软件包进行分析,计量资料用均数±标准差表示,采用方差分析,计数资料采用卡方检验。结果:1.血清TGF-β1在正常对照组、轻度子痫前期组、重度子痫前期组的浓度分别为673.68±25.16ng/L、930.93±30.34ng/L、1090.88±31.37ng/L,血清s Eng在正常对照组、轻度子痫前期组、重度子痫前期组各组中的浓度分别为24.76±3.39ng/ml、38.15±3.15ng/ml、47.50±3.60ng/ml。随着病情的加重,TGF-β1/s Eng浓度呈逐渐上升趋势。与对照组比较,轻度、重度子痫前期组血清TGF-β1/s Eng浓度明显升高,重度子痫前期组血清中TGF-β1/s Eng高于轻度子痫前期组,且差异均具有显著的统计学意义(p0.01)。2.TGF-β1在正常对照组、轻度子痫前期组、重度子痫前期组TGF-β1的阳性率分别是26.67%、76.67%、93.33%,Endoglin在正常对照组、轻度子痫前期组、重度子痫前期组胎盘中表达的阳性率分别为30.00%、73.33%、93.33%,从整体局势可见,病情越重,TGF-β1的表达阳性率越高,成递增的趋势。三组之间两两比较显示:轻度、重度子痫前期组胎盘TGF-β1/Endoglin表达强于正常对照组,差异具有统计学意义(p0.016),而轻度、重度子痫前期组比较,差异无统计学意义(p0.016)。3.各组血清中的s Eng与血清中的TGF-β1之间呈正相关,相关系数r分别为0.75、0.69、0.72。各组胎盘组织中TGF-β1与Endoglin的表达有正相关性,相关系数r分别为0.70、0.65、0.72;各组胎盘组织中TGF-β1与血清中的TGF-β1之间有正相关性,相关系数r分别为0.67、0.73、0.71;各组血清中的s Eng与胎盘中的Endoglin也呈正相关,相关系数r分别为0.75、0.72、0.71。且差异均具有统计学意义(p0.05)。结论:1.血清TGF-β1、s Eng参与了正常妊娠及子痫前期的发生过程。且病情越重,血清TGF-β1、s Eng的表达越高。2.胎盘组织中TGF-β1、Endoglin的过度表达,与子痫前期发病关系密切,随着病情的加重,表达逐渐增加。3.血清s Eng可能是胎盘组织分泌Endoglin的一种可溶性形式,并通过与TGF-β1之间的相互作用共同参与了血液循环中血压的调控及内皮细胞的损害。
[Abstract]:Objective: to detect the concentration of TGF- 尾 1s Eng in peripheral blood of preeclampsia patients and the expression of TGF- 尾 1 Endoglin in placenta of preeclampsia and normal pregnant women, and to compare the expression of TGF- 尾 1 Endoglin and the content of serum TGF- 尾 1s Eng in preeclampsia and normal pregnant women. To explore the role of TGF- 尾 1 endoglins Eng in the etiology and pathogenesis of preeclampsia. Methods: from June 2014 to December 2014, 60 cases of preeclampsia were randomly selected in our hospital (including 30 cases of mild preeclampsia group, 30 cases of mild preeclampsia group). There were 30 cases of severe preeclampsia group and 30 cases of severe preeclampsia group. The diagnostic criteria of preeclampsia referred to the 8th edition of obstetrics and gynecology edited by Xie Xing. 30 cases of late pregnant women with normal blood pressure were selected as control group. There was no significant difference between delivery mode and pregnancy. Serum TGF- 尾 1s Eng concentration was determined by double antibody sandwich enzyme-linked immunosorbent assay (Elisa), and TGF- 尾 1- Endoglin in placenta tissue was detected by immunohistochemical SP method. Using SPSS19.0 software package to analyze, The measured data were expressed as mean 卤standard deviation, ANOVA and chi-square test were used. Results: 1. Serum TGF- 尾 1 in normal control group, mild preeclampsia group, and mild preeclampsia group. The concentrations of serum s Eng in the severe preeclampsia group were 673.68 卤25.16ng / L 930.93 卤30.34ng / L 1090.88 卤31.37ng / L, respectively. The concentrations of serum s Eng in normal control group, mild preeclampsia group and severe preeclampsia group were 24.76 卤3.39ngml / ml 38.15 卤3.15ngml / ml 47.50 卤3.60ng / ml respectively. The concentration of TGF- 尾 1 / s Eng in the mild and severe preeclampsia group was significantly higher than that in the mild preeclampsia group. The serum TGF- 尾 1 / s Eng in the severe preeclampsia group was significantly higher than that in the mild preeclampsia group, and the difference was statistically significant (P < 0.01). 2. TGF- 尾 1 in the normal control group and the mild preeclampsia group. The positive rates of TGF- 尾 1 in the placenta of the severe preeclampsia group were 26.677,76.670.33and 93.33 respectively. The positive rates of TGF- 尾 1 expression in the placenta of the mild preeclampsia group, the mild preeclampsia group and the severe preeclampsia group were respectively 30.0073.33 and 93.33.The positive rate of TGF- 尾 1 expression in the placenta of the severe preeclampsia group was higher than that of the severe preeclampsia group. The expression of TGF- 尾 1 / Endoglin in placenta of mild and severe preeclampsia group was higher than that of normal control group, the difference was statistically significant (p 0.016), but that of mild and severe preeclampsia group was higher than that of mild and severe preeclampsia group. There was no significant difference between the two groups. There was a positive correlation between serum s Eng and TGF- 尾 _ 1 in each group, and the correlation coefficient r was 0.75-0.69 ~ 0.72, respectively. There was a positive correlation between the expression of TGF- 尾 _ 1 and Endoglin in placenta of each group, and the expression of TGF- 尾 _ 1 was positively correlated with the expression of TGF- 尾 _ 1 in placenta of each group. There was a positive correlation between TGF- 尾 _ 1 in placenta and TGF- 尾 _ 1 in serum (r = 0.670.73 卤0.71), and there was a positive correlation between serum s Eng and placenta Endoglin. The correlation coefficient r was 0.75 卤0.72 卤0.71, respectively. The differences were statistically significant (P 0.05). Conclusion: 1. Serum TGF- 尾 1s Eng is involved in the development of normal pregnancy and preeclampsia. The more serious the condition, the higher the expression of TGF- 尾 1s Eng. 2. The overexpression of TGF- 尾 1 Endoglin in placenta. The expression of serum s Eng may be a soluble form of Endoglin secretion in placenta. The interaction with TGF- 尾 1 involved in the regulation of blood pressure and the damage of endothelial cells.
