宫颈癌后装治疗中膀胱充盈状态对膀胱及直肠受量的影响

【摘要】:目的:分析膀胱在充盈和排空状态下宫颈癌后装治疗中对膀胱及直肠受量的影响,为减少宫颈癌后装治疗后出现并发症膀胱炎及直肠炎提供依据。 方法:选取20例完成盆腔外照射的FIGO分期为IIa期-IIIb期的宫颈癌患者,应用荷兰核Fletcher系统给予三管近距离照射,另外选取20例FIGO分期为Ib-IIa期的宫颈癌根治术后患者,,给予二管近距离照射。利用CT模拟定位,分别扫描排尿后及排尿后半小时膀胱充盈50ml以上的CT图像。在膀胱排空状态及充盈状态下的CT图像上勾画危及器官(OAR)膀胱及直肠。分别制定膀胱排空及充盈状态下的治疗计划,计划处方剂量为600cGy/次,根据DVH图评价危及器官剂量情况,以评价出正常组织剂量分布较好的宫颈癌近距离治疗计划。 结果:1、三管近距离照射膀胱在充盈50ml以上的状态下,1cm3体积的膀胱受照剂量B-D1400±70(cGy)明显大于膀胱排空状态下1cm3体积的膀胱受照剂量B-D1332±65(cGy),具有统计学意义(P<0.05);2cm3体积的膀胱受照剂量B-D2362±63(cGy)明显大于2cm3体积的膀胱排空状态下膀胱受照剂量B-D2296±94(cGy),具有统计学意义(P<0.05);直肠受照剂量在膀胱排空及充盈两种状态下无明显差别。 2、二管近距离照射膀胱在充盈50ml以上状态下,1cm3体积的膀胱受照剂量b-D1395±55(cGy)明显大于膀胱排空状态下1cm3体积的膀胱受照剂量b-D1350±76(cGy),具有统计学意义(P<0.05);2cm3体积的膀胱受照剂量b-D2368±59(cGy)明显大于膀胱排空状态下2cm3体积的膀胱受照剂量r-D2315±63(cGy),具有统计学意义(P<0.05);直肠受照剂量在膀胱排空及充盈两种状态下无明显差别。 结论:宫颈癌患者行腔内近距离放疗时,膀胱充盈状态对膀胱受量存在影响。排空膀胱后可较好的保护膀胱,减少膀胱受照剂量。膀胱充盈状态对直肠受量影响不大。
[Abstract]:Objective: to analyze the effect of bladder filling and emptying on bladder and rectum in order to reduce the complications of cystitis and proctitis.Methods: twenty patients with cervical cancer with FIGO staging of stage IIa-IIIb who had completed external pelvic irradiation were treated with three-tube brachytherapy using the Dutch nuclear Fletcher system, and 20 patients with FIGO staging as stage Ib-IIa after radical resection of cervical cancer were selected.Give two tubes of brachytherapy.Ct images of bladder filling with 50ml after urination and half an hour after urination were scanned by CT simulated localization.The bladder and rectum were delineated on CT images under bladder emptying and filling.The treatment plan under the condition of bladder emptying and filling was made, and the planned prescription dose was 600cGy/.
