血清HE4、CA125及二者联合应用对卵巢癌诊断价值的Meta分析

【摘要】:目的:通过Meta分析对血清HE4、CA125及两者联合诊断卵巢癌的价值进行探讨。 方法:通过PubMed、Elsevier Science、Springer、中国知网、万方及维普等数据库检索2006年1月至2013年12月的相关文献,按照纳入标准和排除标准对文献进行严格筛选。提取纳入文献的资料特征及四格表,对纳入研究使用QUADAS评价工具详情表。进行质量评价。使用Meta-Disc软件1.4版进行统计学分析,分析及处理异质性后选择适当效应模型合并效应量,得出合并敏感性、合并特异性及合并似然比的森林图,制作SROC曲线。 结果:初检得到中、英文文献共1046篇,筛选后20篇文献可纳入研究。经Meta分析后得出合并效应量:HE4的敏感度为0.70,特异度为0.92,阳性似然比为8.65,阴性似然比为0.31,SROC曲线下面积0.9116;CA125的敏感度为0.79,特异度为0.80,,阳性似然比为3.97,阴性似然比为0.28,SROC曲线下面积0.8678;两者联合使用的敏感度为0.88,特异度为0.75,阳性似然比为3.62,阴性似然比为0.16,SROC曲线下面积(AUC)为0.9064。 结论:HE4与CA125联合使用对卵巢癌的诊断价值高于分别单独使用,当患者血清HE4和CA125均为阴性时,排除卵巢癌的能力较强。
[Abstract]:Objective: to evaluate the value of serum HE4, CA125 and their combination in diagnosis of ovarian cancer by Meta analysis.Methods: the relevant literatures from January 2006 to December 2013 were searched through the database of PubMedus Elsevier Science Spring, China Zhiwang, Wanfang and Weip, and the literature was screened strictly according to the inclusion criteria and exclusion criteria.The data features and four grid tables of the literature were extracted, and the QUADAS evaluation tool was used for the inclusion study.Carry out quality evaluation.Statistical analysis was carried out by using Meta-Disc software version 1.4. After analyzing and dealing with heterogeneity, appropriate effect model was selected to combine effect quantity, and the forest map of combination sensitivity, combination specificity and merge likelihood ratio were obtained, and SROC curves were made.Results: a total of 1046 Chinese and English literatures were obtained, 20 of which could be included in the study.Meta analysis showed that the sensitivity, specificity, positive likelihood ratio, negative likelihood ratio under the SROC curve were 0.70, 0.92, 8.65, 0.79, 0.80, 3.97 and 0.28, respectively.The sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and area under SROC curve were 0.88, 0.75, 3.62 and 0.9064, respectively.Conclusion the diagnostic value of the combination of CA125 and He4 for ovarian cancer is higher than that used separately. When the serum HE4 and CA125 are both negative, the ability to exclude ovarian cancer is stronger.
