DWI和CE-MRI评价高强度聚焦超声刀（HIFU）治疗子宫肌瘤术后早期的疗效价值比较

【摘要】：目的：探讨DWI和CE-MRI在评价高强度聚焦超声刀(HIFU)治疗子宫肌瘤术后早期的疗效价值对比。方法：18例患者共48个子宫肌瘤病灶在HIFU治疗术前及术后一天分别行常规MRI、DWI检查及CE-MRI检查,分别分析术前、术后消融区及残留区DWI信号特征及ADC值差异,并比较DWI和CE-MRI所测的肌瘤的消融率。结果：48个子宫肌瘤在HIFU治疗后一天DWI和CE-MRI均能不同程度的发现瘤体残留区。HIFU术后一天肌瘤消融区平均ADC值,较术前有降低,且差异有显著性统计学意义(P0.001)；HIFU术后一天肌瘤残留区平均ADC值,较术前有升高,且差异有显著性统计学意义(P0.001); DWI和CE-MRI对术后一天子宫肌瘤的平均消融率,统计学差异无意义(P0.05)。结论：1、DWI早期评价HIFU治疗子宫肌瘤疗效是非常有效的方法；2、DWI和CE-MRI在早期评价HIFU治疗子宫肌瘤术后疗效方面无明显差异。
[Abstract]:Objective: to evaluate the efficacy of DWI and CE-MRI in the treatment of uterine leiomyoma with high intensity focused ultrasound (HIFU). Methods Forty-eight uterine leiomyoma lesions in 18 patients were examined with conventional Mr DWI and CE-MRI before and one day after HIFU. The characteristics of DWI signal and the difference of ADC in ablation and residual areas before and after operation were analyzed respectively. The ablation rate of leiomyoma measured by DWI and CE-MRI was compared. Results: the mean ADC value of 48 uterine leiomyomas on one day after HIFU treatment in DWI and CE-MRI could be found in different degree. The mean value of ADC in the ablation area of uterine leiomyoma on one day after operation was lower than that before operation. The difference was statistically significant (P 0.001). The mean ADC value in the residual area of leiomyoma one day after operation was higher than that before operation, and the difference was statistically significant (P 0.001). There was no significant difference between DWI and CE-MRI in the mean ablation rate of uterine leiomyoma one day after operation (P 0.05). Conclusion early evaluation of the effect of HIFU on uterine leiomyoma is a very effective method. There is no significant difference between the early evaluation of HIFU and CE-MRI in the treatment of uterine leiomyoma.
