【摘要】:目的:本研究旨在通过检测HPV感染和不同程度的宫颈病变(CIN2-3)患者阴道冲洗液的细胞因子(IL-2)和一氧化氮(NO)的水平,探讨IL-2、NO和CIN之间是否相关,IL-2和NO的趋势,阴道局部免疫与CIN的关系,为预防宫颈癌的发生提供了可能。 方法:收集2012-2013年大连市妇幼保健院妇科门诊收治的18-65岁并且阴道镜下活检确诊为HPV感染的患者30例,阴道镜下活检确诊为HPV感染同时为CIN2-3的患者30例,同期正常体检的健康妇女30例作对照组。剔除标准:妊娠及哺乳期,服用性激素妇女;阴道病变,阴道流血,l周内有阴道用药史,3天内性生活史;有长期使用皮质类固醇激素及免疫抑制剂用药史者;2周内全身使用抗生索的患者。标本的采集均经研究对象的知情同意。本实验通过酶联免疫吸附试验法即ELISA法来检测HPV、CIN2-3及正常健康妇女阴道局部细胞因子IL-2、NO的水平。全部数据应用SPSS13.0统计软件进行数据处理分析。 结果: 1.两组一般情况的比较HPV组、HPV+CIN2-3组及对照组,三组妇女在年龄、怀孕次数、性伴侣个数、第一次性生活年龄、吸烟、口服避孕药以及每年体检(TCT)、家族遗传相比较,(p0.05)差异均无统计学意义。 2. HPV组与对照组比较:IL-2水平明显降低(P0.01);NO水平明显上升(P0.01) 3.CIN2-3组与对照组比较:IL-2水平明显降低(P0.01);NO水平明显上升(P0.01)。 4.CIN2-3组与HPV组比较:IL-2水平下降(P0.05),NO水平上升(P0.05)。 结论: CIN虽属于连续发展病变,但也存在极大逆转可能,且CIN发展为宫颈癌常需要8-10年。因此,深刻认识免疫因素在CIN转归中作用及微量元素与机体免疫状态关系,可为CIN和宫颈癌的辅助治疗、预防开拓新视野,提供新的研究方向和治疗思路。本实验证实了:1.IL-2和NO与CIN相关。2.随着宫颈病变的发展IL-2呈下降趋势,NO呈上升趋势。3.阴道局部免疫功能下降是CIN发生,,发展及演变的重要因素之一。
[Abstract]:Objective: to investigate the relationship between IL 2 no and CIN by detecting the levels of cytokine 2 (IL 2) and nitric oxide (no) in vaginal lavage fluid of patients with HPV infection and cervix cervicitis (CIN2-3), and to explore whether there is a correlation between IL 2 no and CIN. The relationship between vaginal local immunity and CIN provides the possibility for the prevention of cervical cancer. Methods: thirty patients aged 18-65 years with HPV infection diagnosed by colposcopy biopsy and 30 patients with HPV infection and CIN2-3 diagnosed by colposcopy biopsy were collected from the gynecological outpatient department of Dalian Maternal and Child Health Hospital from 2012 to 2013. At the same time, 30 healthy women who had normal physical examination served as control group. Exclusion criteria: pregnancy and lactation, taking sex hormone for women, vaginopathy, vaginal bleeding within 1 week history of vaginal medication and sexual history within 3 days; Patients with a long history of corticosteroid use and immunosuppressant use throughout the body within 2 weeks. All specimens were collected with the informed consent of the subjects. In this study, the levels of CIN2-3 and IL-2no in vaginal area of healthy women were detected by enzyme linked immunosorbent assay (ELISA). All data are processed and analyzed by SPSS13.0 statistical software. Results: 1. Comparison of the general conditions between the two groups: HPV CIN2-3 group and control group, three groups of women in age, number of pregnancies, number of sexual partners, first life age, smoking, There was no significant difference in family heredity between oral contraceptive pills and annual physical check-up (P 0.05). 2. Compared with the control group, the level of IL-2 in the HPV group was significantly lower than that in the control group. Compared with the control group, the level of IL-2 in the 3.CIN2-3 group was significantly lower than that in the control group. Compared with HPV group, the level of IL-2 in 4.CIN2-3 group was lower than that in HPV group, and the level of no in 4.CIN2-3 group was higher than that in HPV group. Conclusion: Although CIN is a progressive lesion, it may be reversed greatly, and it usually takes 8 to 10 years for CIN to develop into cervical cancer. Therefore, a profound understanding of the role of immune factors in the outcome of CIN and the relationship between trace elements and immune status of the body can provide a new research direction and treatment ideas for the adjuvant treatment of CIN and cervical cancer, the development of new horizons for prevention and treatment. This experiment confirmed that: 1. IL-2 and no are related to CIN. With the development of cervical lesions, IL-2 showed a downward trend. The decrease of vaginal local immune function is one of the important factors in the occurrence, development and evolution of CIN.
