孕期体重管理的实验研究

【摘要】:孕妇孕期体重增长过快、增长过多成为当今社会普遍存在的问题,也直接导致许多不良妊娠结局的发生。孕期体重适宜增长对促进胎儿生长发育、减少产妇分娩并发症、促进产后母体恢复及子代远期健康都极为重要。为了保证孕期体重稳定、科学地增长,应进行孕期体重管理。孕期体重管理包括孕期营养管理、运动管理及良好生活方式的管理三个方面,其通过对孕妇孕期营养、运动及生活方式进行干预,使孕期体重增长在与其身高、基础体重相符,让孕妇有更好的妊娠结局,让婴儿的出生体重在合理区间。孕期合理的饮食能有效控制孕期体重已被证实,笔者拟通过本研究,探讨孕期运动对控制孕期体重的作用,以及孕期体重对分娩方式及胎儿体重的影响。并在此过程中,研发强度适宜、安全可行的孕期运动。本文主要采用了实验法、数据分析法、文献资料法、观察法等研究方法,随机抽取在四川省妇幼保健院建卡、定期产检的孕妇200名为实验对象,以她们的孕期体重管理为研究对象。随机将孕妇分为干预组和对照组。干预组在常规产检的基础上同时接受营养科的营养规划及产科的孕期生活指导,并定期在四川省妇幼保健院参加孕期运动20次以上。同时,将干预组根据孕前BMI的不同情况,分为3个组别,对这三个组别的孕妇施以不同的运动时间、频率及略有区别的运动强度。对照组在常规产检的基础上只接受营养科的营养规划及产科的孕期生活指导,不参与四川省妇幼保健院的孕期运动。实验结束后,将实验对象的年龄、身高、孕前体重、分娩时体重、分娩方式、婴儿出生体重及孕期体重阶段性增长情况进行汇总,并采用SPSS17.0统计软件分析数据,计量资料以均数±标准差(±S)表示,两组计量资料比较采用t检验,两组率的比较采用检验。以P0.05表示差异有统计学意义。研究表明:1.干预组里孕前正常和低BMI的孕妇孕期体重增加在推荐范围内。2.干预组里孕前正常BMI的孕妇顺产率最高。3.干预组里婴儿出生体重98%在正常区间。4.干预组分娩前体重和孕期体重增长低于对照组。5.干预组婴儿出生体重在正常区间的比例高于对照组。6.干预组顺产比例高于对照组。
[Abstract]:Too fast weight gain during pregnancy has become a common problem in today's society, which directly leads to the occurrence of many adverse pregnancy outcomes. Appropriate weight gain during pregnancy is very important for promoting fetal growth and development, reducing parturient complications, promoting postpartum maternal recovery and long-term health of offspring. In order to ensure stable weight and scientific growth during pregnancy, weight management during pregnancy should be carried out. The weight management during pregnancy includes nutrition management during pregnancy, exercise management and good lifestyle management. By intervening in the nutrition, exercise and lifestyle of pregnant women, the weight gain during pregnancy is consistent with their height and basic weight. Let pregnant women have better pregnancy outcome, let the baby's birth weight in a reasonable range. It has been proved that a reasonable diet during pregnancy can effectively control the weight during pregnancy. The author intends to explore the effect of exercise during pregnancy on the control of weight during pregnancy and the influence of weight during pregnancy on the delivery mode and fetal weight. And in this process, research and development intensity is appropriate, safe and feasible pregnancy exercise. In this paper, 200 pregnant women who were randomly selected from Sichuan Maternal and Child Health Hospital to build cards and regular birth tests were selected as experimental objects by means of experimental method, data analysis method, literature method, observation method and so on. Their weight management during pregnancy was studied. Pregnant women were randomly divided into intervention group and control group. On the basis of routine birth examination, the intervention group received nutrition planning from nutrition department and guidance of life during pregnancy in obstetrics department, and regularly participated in pregnancy exercise more than 20 times in Sichuan Maternal and Child Health Hospital. At the same time, the intervention group was divided into three groups according to the different conditions of pre-pregnancy BMI. The pregnant women in these three groups were given different exercise time, frequency and slightly different exercise intensity. On the basis of routine birth examination, the control group only received nutrition planning of nutrition department and guidance of life during pregnancy in obstetrics, and did not participate in pregnancy movement of Sichuan Maternal and Child Health Hospital. At the end of the experiment, the age, height, weight before pregnancy, weight during delivery, delivery mode, birth weight and pregnancy weight of the subjects were summarized, and the data were analyzed by SPSS17.0 software. The measurement data were expressed as mean 卤standard deviation (卤S). T test was used to compare the two groups of metrological data and test was used to compare the rates of the two groups. The difference was statistically significant with P0.05. The study shows that 1: 1. Pregnancy weight gain in the intervention group with normal pre-pregnancy and low BMI was within the recommended range. Pregnant women with normal BMI before pregnancy in the intervention group had the highest rate of spontaneous delivery. 3. In the intervention group, 98% of the birth weight of the baby was in the normal range. 4. The weight gain before delivery and during pregnancy in the intervention group was lower than that in the control group. The proportion of birth weight in normal range in intervention group was higher than that in control group. The proportion of spontaneous labor in intervention group was higher than that in control group.
