自拟补肾疏肝汤结合西药治疗肾虚肝郁型早期先兆流产的临床研究

【摘要】：目的:本文主要通过对早期先兆流产的治疗现状及相关临床病例研究,探讨自拟补肾疏肝汤结合西药治疗早期先兆流产的临床综合疗效,以期利用中医药治疗早期先兆流产的优势和特色,将该种治疗方法更多地应用到以后临床保胎工作中。方法:将符合妊娠早期肾虚肝郁型先兆流产诊断标准的68例患者,随机分为治疗组与对照组两组,治疗组患者口服中药自拟补肾疏肝汤并结合西药肌肉注射黄体酮注射液;对照组患者仅用西药肌肉注射黄体酮注射液。每7天为一个疗程,连续治疗2个疗程后观察两组患者临床症状的改善情况、血清激素水平(孕酮、人绒毛膜促性腺激素、雌二醇)及B超监测妊娠囊的生长的检验结果,综合分析其结果对两组临床疗效的影响。结果:治疗组34例患者,总有效率94.12%,痊愈8例(23.53%),显效17例(50.00%),有效7例(20.59%),无效2例(0.06%);对照组34例患者,总有效率82.35%,痊愈5例(14.71%),显效8例(23.53%),有效15例(44.12%),无效6例(17.65%)。中西药联合用药的治疗组临床疗效明显优于单纯西药治疗的对照组(P0.05),且治疗组接受治疗后的血清孕酮(P)、血清绒毛膜促性腺激素(β-HCG)及血清雌二醇(E2)比用药前有明显的提高,妊娠囊比用药前明显有生长。结论:自拟补肾疏肝汤联合黄体酮治疗肾虚肝郁型早期先兆流产疗效明显优于临床单独应用黄体酮,具体表现在改善孕期临床症状、提高血清P、β-HCG、E2水平和胎儿在宫内正常发育情况及临床疗效评价方面。因此中西医结合治疗早期先兆流产能陪同保护患者安全顺利度过妊娠期。
[Abstract]:Objective: to explore the clinical comprehensive effect of Bushen Shugan decoction combined with western medicine in the treatment of early threatened abortion. In order to make use of the advantages and characteristics of Chinese medicine to treat early threatened abortion, this treatment method will be applied more to the later clinical fetal preservation work. Methods: 68 cases of threatened abortion with kidney deficiency and liver depression in early pregnancy were randomly divided into two groups: the treatment group and the control group. The patients in the treatment group were treated by oral Chinese medicine Bushen Shugan decoction combined with western medicine intramuscular injection of progesterone injection. Patients in the control group were treated by intramuscular injection of progesterone. The improvement of clinical symptoms, serum hormone levels (progesterone, human chorionic gonadotropin, estradiol) and B-ultrasound monitoring of the growth of pregnancy sac were observed after two consecutive courses of treatment. Comprehensive analysis of the results of the two groups of clinical efficacy. Results: in the treatment group, the total effective rate was 94.12. The cure rate was 23.53 in 8 patients, the significant effect was 50.005 in 17 patients, the effective rate was 20.599.59 in 7 patients, the ineffective in 2 patients was 0.060.The total effective rate was 82.35 in the control group, the total effective rate was 82.35, the cure rate was 14.71, the significant effect was 23.535.The effective rate was 44.12, and the ineffective was 17.656.The total effective rate was 82.35 in the control group. The clinical efficacy of the combined treatment group was significantly better than that of the control group (P 0.05), and the serum progesterone, chorionic gonadotropin (尾 -HCG) and serum estradiol E _ 2 (E _ 2) of the treatment group were significantly higher than those of the control group. The pregnancy sac grew more obviously than before. Conclusion: the effect of Bushen Shugan decoction combined with progesterone in the treatment of early threatened abortion with kidney deficiency and liver depression is better than that of progesterone alone, which is manifested in the improvement of clinical symptoms during pregnancy. To improve the level of serum P- 尾 -HCGG E 2 and the normal development of fetus in uterus and the evaluation of clinical curative effect. Therefore, integrated Chinese and western medicine treatment of early threatened abortion can be accompanied to protect the patient safely and smoothly through pregnancy.
