Twist与KiSS-1在宫颈鳞癌中的表达及临床意义的研究

【摘要】:目的:通过检测Twist和KiSS-1在正常宫颈、宫颈上皮内瘤变(CIN)及宫颈鳞癌(squamous cervical cancer,SCC)组织中的表达情况,探讨二者在宫颈鳞癌发生发展中的作用及其意义。方法:采用免疫组织化学Envison法检测12例正常宫颈(normal cervical epithelium,NCE),20例宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)及47例宫颈鳞癌组织中Twist和KiSS-1的表达情况,分析二者的表达与宫颈癌患者各临床病理因素的相关性。采用χ2检验或Fisher’s精确概率法比较计数资料间的差异,采用Spearman法进行相关性检验,生存曲线采用Kaplan-Meier,生存率比较采用Log-rank检验,COX比例风险模型进行多因素分析。结果:1、Twist在正常宫颈、CIN中主要表达于细胞核,而在SCC中主要表达于细胞质中。Twist在正常宫颈、CIN、SCC中的表达呈逐渐上升趋势,其阳性表达率分别为:16.67%(2/12)、30%(6/20)、85.11%(40/47),三组间比较,差异有统计学意义(χ2=29.407,P=0.0000.05);Twist在SCC中的阳性表达率与正常宫颈组中的阳性表达率比较,差异有统计学意义(χ2=21.830,P=0.0000.05);Twist在SCC组织中的阳性表达率与CIN中的阳性表达率比较,差异有统计学意义(χ2=19.798,P=0.0000.05)。Twist在宫颈癌患者不同临床特征组中,如患者年龄、FIGO分期、组织分化程度、淋巴结转移、间质浸润深度等的表达差异无统计学意义(P0.05),但有淋巴结转移者比无转移者有较高的Twist表达,其中无淋巴结转移者阳性表达为81.08%,有淋巴结转移者为100%,经统计学分析,χ2=2.223,P=0.3180.05,其差异无统计学意义。2、KiSS-1在宫颈上皮、SCC中主要表达于细胞膜和细胞质,胞核中亦有少量表达。KiSS-1在正常宫颈组、CIN组、SCC组中的表达呈逐渐下降的趋势,其阳性表达率分别为:91.67%(11/12)、70.00%(14/20)、27.7%(13/47),,三组间比较,差异有统计学意义(χ2=20.832,P=0.0000.05);KiSS-1在SCC组中的阳性表达率与正常宫颈组中的阳性表达率比较,差异有统计学意义(χ2=38.417,P=0.0000.05); KiSS-1在SCC组中的阳性表达率与CIN组中的阳性表达率比较,差异有统计学意义(χ2=29.987,P=0.0000.05)。KiSS-1在宫颈癌不同临床特征组中,如患者年龄、FIGO分期、组织分化程度、淋巴结转移、间质浸润深度等的表达差异均无统计学意义(P0.05)。3、Twist和KiSS-1在宫颈鳞癌组织中的表达无相关性(rs=0.027,P=0.8590.05)。4、单因素及多因素分析结果显示Twist及KiSS-1的表达与宫颈鳞癌患者的预后无关(P值均0.05)。结论:1. Twist在正常宫颈组织、CIN及宫颈鳞癌组织中的表达呈依次升高趋势,说明Twist可能与宫颈鳞癌的发生发展有关。Twist在宫颈鳞癌中的表达与年龄、FIGO分期、组织学分级、间质浸润深度及有无淋巴结转移等临床病理因素无关。2.KiSS-1在正常宫颈组织、CIN及宫颈鳞癌组织中的表达呈依次降低趋势,说明KiSS-1的低表达与宫颈鳞癌的发生发展可能相关。KiSS-1在宫颈鳞癌中的表达与年龄、FIGO分期、组织学分级、间质浸润深度及有无淋巴结转移等临床病理因素无关。3. Twist的表达及KiSS-1的表达与宫颈鳞癌患者的预后无关。4. Twist及KiSS-1在宫颈鳞癌中的表达无相关性。
[Abstract]:Objective: To investigate the role and significance of the expression of Twist and KiSS-1 in the normal cervix, cervical intraepithelial neoplasia (CIN) and squamous cervical cancer (SCC), and to explore the role and significance of the two cases in the development of cervical squamous cell carcinoma. Methods: 12 cases of normal cervix (normal cervical epith) were detected by immunohistochemical Envison method. Elium, NCE), the expression of Twist and KiSS-1 in 20 cases of cervical intraepithelial neoplasia (cervical intraepithelial neoplasia, CIN) and 47 cases of cervical squamous cell carcinoma. The correlation between the expression of the two and the clinicopathological factors of the patients with cervical cancer was analyzed. The difference between the count data was compared by the chi 2 test or the Fisher 's precision method, and Spearman was used. The correlation test was carried out, the survival curve was Kaplan-Meier, the survival rate was compared with the Log-rank test and the COX proportional risk model was used for multiple factors analysis. Results: 1, Twist was in the normal cervix and CIN was mainly expressed in the nucleus, while the expression of.Twist in the cytoplasm was gradually rising in the normal cervix, CIN, and SCC in the cytoplasm. The positive expression rates were 16.67% (2/12), 30% (6/20) and 85.11% (40/47). The difference between the three groups was statistically significant (x 2 = 29.407, P=0.0000.05), and the positive rate of Twist in SCC was compared with the positive rate in the normal cervix group (x 2 = 21.830, P=0.0000.05), and Twist was positive in SCC tissues. Compared with the positive expression rate in CIN, the difference was statistically significant (x 2 = 19.798, P=0.0000.05).Twist in different clinical characteristics of cervical cancer patients, such as patients' age, FIGO staging, degree of differentiation, lymph node metastasis, interstitial infiltration depth, and so on (P0.05), but there was no transfer of lymph node metastases. The positive expression of the Twist was 81.08% and the lymph node metastasis was 100%, and the difference was 2.223, P=0.3180.05. The difference was not statistically significant.2, KiSS-1 was in the cervical epithelium, SCC was mainly expressed in the cell membrane and the cell cytoplasm, and a small amount of.KiSS-1 was expressed in the normal cervix group. In group CIN, the expression in group SCC showed a trend of gradual decline. The positive expression rates were 91.67% (11/12), 70% (14/20) and 27.7% (13/47). The difference was statistically significant (x 2 = 20.832, P=0.0000.05), and the positive rate of KiSS-1 in the SCC group was compared with the positive rate in the normal cervical group, the difference was statistically significant. 2 = 38.417, P=0.0000.05); the positive expression rate of KiSS-1 in group SCC compared with the positive expression rate in group CIN was statistically significant (x 2 = 29.987, P=0.0000.05) in different clinical features of cervical cancer, such as patients' age, FIGO stage, tissue differentiation, lymph node metastasis, interstitial infiltration depth and so on Statistical significance (P0.05).3, Twist and KiSS-1 expression in cervical squamous cell carcinoma (rs=0.027, P=0.8590.05).4. Single factor and multivariate analysis showed that the expression of Twist and KiSS-1 was not related to the prognosis of cervical squamous cell carcinoma (P value was 0.05). Conclusion: 1. Twist in normal cervical tissues, CIN, and cervical squamous cell carcinoma tissue The trend in turn indicates that Twist may be associated with the development of cervical squamous cell carcinoma and the expression and age of.Twist in cervical squamous cell carcinoma, FIGO staging, histological grading, interstitial infiltration depth and lymph node metastasis are not related to.2.KiSS-1 in normal cervical tissue, and the expression of CIN and cervical squamous cell carcinoma is decreasing in turn. The expression of KiSS-1 and the development of cervical squamous cell carcinoma may be related to the expression and age of.KiSS-1 in cervical squamous cell carcinoma, FIGO staging, histological grading, interstitial infiltration depth and lymph node metastasis are not related to the expression of.3. Twist and the expression of KiSS-1 and the prognosis of cervical squamous carcinoma is not related to.4. Twist and KiSS-1. There is no correlation in the expression in squamous cell carcinoma of the cervix.
