骨代谢标志物在不同妊娠次数中对胎儿长骨发育的影响

【摘要】:目的通过测定初次妊娠及二次妊娠妇女骨代谢标志物、骨密度值的水平了解孕妇的骨骼状态及同期胎儿长骨的发育情况,分析妊娠次数是否会对胎儿长骨及孕妇自身骨密度产生影响。方法随机选取2016年2月至2016年12月在宁夏医科大学总医院就诊的妊娠妇女520例,根据排除标准排除16例,剩余504例作为研究对象。根据妊娠次数将妊娠妇女分为两组,初次妊娠455例,二次妊娠49例。分别对两组的骨代谢标记物、胎儿长骨数值进行比较分析。其中产后1年出现腰酸背痛、腓肠肌痉挛的产妇共104例,初次妊娠91例,二次妊娠13例,比较两组妊娠前、产后3个月、产后1年的骨密度值。结果1.二次妊娠组的年龄均大于初次妊娠组,差异具有统计学意义(P0.05),而孕周在两组间无显著性差异(P0.05)。2.初次妊娠组、二次妊娠组的维生素D水平均小于正常值(≤30ng/ml),两组维生素D水平相比较,无显著性差异(P0.05)。3.初次妊娠组、二次妊娠组的血清钙、磷、镁水平相比较,无显著性差异(P0.05)。4.初次妊娠组的CTX-1、PINP水平与二次妊娠组相比较,无显著性差异(P0.05)。5.初次妊娠组、二次妊娠组组间比较胎儿股骨值,无显著性差异(P0.05)。6.二次妊娠组的妊娠前、产后3个月、产后1年的骨密度值均较初次妊娠组略有减低,两组比较存在显著性差异(P0.05)。结论1.妊娠次数对孕妇的维生素D、Ca、P、Mg、CTX-1、PINP的水平及胎儿长骨发育无明显影响。2.妊娠次数对孕妇妊娠自身骨密度值有一定的影响,且产后3个月,孕妇的骨密度值处于最低水平。
[Abstract]:Objective to study the bone status of pregnant women and the development of fetal long bones in the same period by measuring the biomarkers of bone metabolism and bone mineral density in women with first and second pregnancies. To analyze whether the number of pregnancies will have an effect on fetal bone and pregnant women's own bone mineral density. Methods from February 2016 to December 2016, 520 pregnant women were randomly selected from the General Hospital of Ningxia Medical University.
