人滋养细胞表面抗原(Trop-2)在宫颈病变中表达的研究

【摘要】:在妇科最常见的恶性肿瘤中,宫颈癌是其中之一,全世界每年宫颈癌新增病例约529800,死亡病例约275100,其中发展中国家每年新增约453300,死亡病例约242000,,占全世界的85%以上[1]。宫颈上皮内瘤变(cervical intraepizhelialneoplasia,CIN)是与子宫颈浸润癌密切相关的一组子宫颈病变,具有两种不同结局:一是病变自然消退;二是病变发展,进展为浸润癌[2]。Trop-2由定位于1P32上的TACSTD2(肿瘤相关钙信号转导基因2)编码[4]、由323个氨基酸构成的的单次跨膜糖蛋白[3],为一类钙离子通道信号转化器,但其信号转导的具体机制仍不清楚[5]。目前国内尚未见Trop-2与宫颈病变关系的临床研究报道,本研究用RT-PCR、免疫组织化学等技术检测Trop-2在宫颈病变中的表达,探讨其与宫颈病变临床病理的关系。 目的观察并探讨Trop-2在人宫颈病变中的表达及其临床相关性。方法运用RT-PCR的方法检测6例正常宫颈组织、6例CIN Ⅰ或CIN Ⅱ组织、6例CINⅢ组织和6例宫颈癌组织中Trop-2mRNA的表达情况;免疫组织化学方法检测Trop-2蛋白在20例正常宫颈组织、20例CIN Ⅰ或CIN Ⅱ组织、20例CINⅢ组织及20例宫颈癌组织中的表达情况。 结果1、Trop-2mRNA在上述的宫颈病变组织中均有表达,且随着病变程度的加重,表达量越多,Trop-2mRNA表达水平随着宫颈病变程度的逐步加深而依次升高。正常宫颈组、CINⅠCINⅡ组、CINⅢ组、宫颈癌组中Trop-2mRNA的平均灰度值分别为0.52,0.76,1.00,1.55,各组之间差异均具有统计学意义(P0.01)。2、Trop-2蛋白在CIN组织及宫颈癌组织中呈阳性表达,主要定位在细胞膜上,且表达水平随着宫颈病变程度的加重而依次升高(P0.01)。在低度上皮内瘤变组织中,表皮基底层以上三分之一细胞可见连续性浅棕色着色,高度上皮内瘤变组织表皮基底层三分之二及上皮全层亦见连续性浅棕色着色,而在正常宫颈组织上皮未见浅棕色着色,个别组织呈现黄色,但并不连续;3、宫颈癌组中,Trop-2表达有19例,强阳性表达有15例,阳性表达3例,弱阳性表达1例,阴性表达1例;在CINⅠ和CINⅡ组中,呈阳性表达的共有16例,其中强阳性表达2例,阳性表达4例,弱阳性表达10;在CINⅢ组中,呈阳性表达的总有18例,其中强阳性表达3例,阳性表达12例,弱阳性表达3例,阴性表达2例。而在正常宫颈组织中,Trop-2弱阳性表达2例,阴性表达18例,随着宫颈病变程度的加深,呈阳性或强阳性表达的例数越多。 结论1、Trop-2的高表达可能参与宫颈病变的发生发展;2、Trop-2或可作为宫颈癌临床筛查及诊断的一个新的标记物3、Trop-2有望成为靶向治疗宫颈癌的一个靶点。
[Abstract]:Among the most common gynecologic malignancies, cervical cancer is one of them. About 529800 of the new cases of cervical cancer are newly added in the world, about 275100 of the deaths in the world, of which about 453300 of the developing countries are added each year, and about 242000 of the deaths, accounting for more than 85% [1]. cervical intraepithelial neoplasia (cervical intraepizhelialneoplasia, CIN) from the cervix of the world. A group of cervical lesions closely related to invasive cancer has two different outcomes: one is the natural degeneration of the lesion, and the two is the development of the lesion, and the progression is that the [2].Trop-2 of the invasive cancer is encoded by the TACSTD2 (tumor related calcium signal transduction gene 2) located on the 1P32, and the single transmembrane glycoprotein [3], which is constructed by 323 amino acids, is a class of calcium ions. The specific mechanism of signal transduction, however, is still not clear in the specific mechanism of signal transduction. There is no clinical study on the relationship between Trop-2 and cervical lesions at present in [5].. This study uses RT-PCR, immunohistochemistry and other techniques to detect the expression of Trop-2 in cervical lesions, and to explore the relationship with the clinicopathology of cervical disease.
Objective To observe and explore the expression and clinical relevance of Trop-2 in human cervical lesions. Methods RT-PCR was used to detect the expression of Trop-2mRNA in 6 normal cervical tissues, 6 CIN I or CIN II tissues, 6 CIN III tissues and 6 cervical cancer tissues, and 20 normal cervical groups were detected by immunohistochemical method. Weave 20 cases of CIN I or CIN II tissue, 20 cases of CIN III and 20 cases of cervical cancer.
Results 1, Trop-2mRNA was expressed in all the cervical lesions, and the more the expression was increased with the severity of the lesion, the level of Trop-2mRNA expression increased with the gradual deepening of the degree of cervical lesions. The average gray value of Trop-2mRNA in the normal cervix group, the CIN I CIN II group, the CIN III group and the cervical cancer group was 0.52,0.76,1.00,1., respectively. 55, the difference between each group was statistically significant (P0.01).2, Trop-2 protein was positive in the CIN tissue and cervical cancer tissue, mainly located on the cell membrane, and the expression level increased in turn (P0.01) with the aggravation of the degree of cervical lesions. In the low degree intraepithelial neoplasia, the epidermis above the basal layer of the 1/3 cells can be seen. Continuous light brown coloring, 2/3 of the epidermis basal layer of the high intraepithelial neoplasia and the epithelia also showed continuous light brown color, but no light brown color was found in the normal cervical tissue epithelium, and some tissues were yellow, but not continuous. 3, 19 cases of Trop-2 were expressed in the cervical cancer group, 15 cases were strongly positive and 3 cases were positive. There were 1 cases of weak positive expression and negative expression in 1 cases. There were 16 positive expressions in group CIN I and CIN II, of which 2 cases were strongly positive, 4 cases were positive and 10 were weakly positive. There were 18 positive expressions in group CIN III, of which 3 were strongly positive, 12 was positive, 3 cases were positive, 3 negative expression, and 2 cases negative. In the cervical tissue, Trop-2 was weakly positive in 2 cases and negative in 18 cases. With the degree of cervical lesions increasing, the number of positive or strong positive cases increased.
Conclusion 1, high expression of Trop-2 may be involved in the development of cervical lesions, and 2, Trop-2 may serve as a new marker for the clinical screening and diagnosis of cervical cancer, and Trop-2 is expected to be a target for targeted treatment of cervical cancer.
