质膜修复蛋白和半胱氨酸天冬氨酸蛋白酶-3在妊娠期高血压疾病胎盘组织中的表达及其意义

【摘要】:目的:利用组织学观察妊娠期高血压疾病患者胎盘的形态学特征,利用免疫组织化学SP法研究质膜修复蛋白(dysferlin)及半胱氨酸天冬氨酸蛋白酶-3(caspase-3)在胎盘的表达,以探讨其与妊娠期高血压疾病发病的关系。 方法:随机选择正常妊娠患者45例,妊娠期高血压患者15例,轻度子痫前期患者15例,重度子痫前期患者15例,制作光镜切片,进行常规HE染色,观察各组胎盘的形态学改变。借助免疫组织化学SP法,应用dysferlin及caspase-3抗体,定位dysferlin和casapse-3在胎盘的表达及其在各组的表达差异。 结果:1、光镜下观察常规HE切片:正常胎盘的合体滋养细胞位于绒毛表层,细胞核排列较整齐,细胞结构一致。妊娠期高血压患者胎盘的滋养细胞增多,可见闭塞性血管内膜炎、绒毛血管灌注减少,纤维蛋白样坏死绒毛增多、合体结增多。 2、Dysferlin在各组的胎盘中均有表达,主要分布于胎盘合体滋养细胞膜。实验组的Dysferlin在合体滋养细胞膜的表达强度低于对照组,,重度子痫前期组(2.07±1.83)、轻度子痫前期组(4.67±1.95)与对照组(6.64±2.14)相比,存在显著差异(P0.05),妊娠期高血压组(5.60±1.45)与对照组相比无显著差异(P0.05)。 3、Caspase-3在各组的胎盘中均有表达,主要分布于胎盘合体滋养细胞浆及部分细胞核。实验组的Caspase-3在合体滋养细胞膜的表达强度高于对照组,重度子痫前期组(7.20±1.74)、轻度子痫前期组(5.93±1.28)和妊娠期高血压组(5.33±1.45)与对照组(2.64±2.25)相比,存在显著差异(P0.05)。 4、Dysferlin和Caspase-3在胎盘中表达呈负相关,相关系数r=-0.4167(P0.001)。 结论:1、Dysferlin在胎盘的合体滋养细胞膜表达,并随着妊娠期高血压疾病病情的加重表达强度减弱。 2、Caspase-3在胎盘的合体滋养细胞的细胞浆及细胞核表达,并随着妊娠期高血压疾病病情的加重表达强度增强。 3、Dysferlin和Caspase-3在胎盘的合体滋养细胞的表达呈负相关性,两者的相互影响可能与妊娠期高血压疾病的发生发展有关。
[Abstract]:Objective: To observe the morphological characteristics of placenta in patients with pregnancy induced hypertension, and to study the expression of plasmalemma repair protein (dysferlin) and cysteine aspartic proteinase -3 (caspase-3) in placenta by immunohistochemical SP method in order to explore the relationship between the placenta and the pathogenesis of hypertensive disorder complicating pregnancy.
Methods: 45 cases of normal pregnancy, 15 cases of pregnancy induced hypertension, 15 cases of mild preeclampsia, 15 cases of severe preeclampsia, 15 cases of severe preeclampsia were made by light microscopy, routine HE staining was made to observe the morphological changes of the placenta in each group. With the help of immunohistochemical SP method, dysferlin and caspase-3 antibody were used to locate dysferlin and CASAP. The expression of se-3 in placenta and its difference in each group.
Results: 1, the normal HE sections were observed under light microscope: the normal placental syncytiotrophoblast was located in the surface of the villi, the nuclei arranged neatly and the cell structure was the same. The placental trophoblast cells of the patients with pregnancy induced hypertension were increased, the obliterans endocarditis, the decrease of villi blood vessel perfusion, the increase of fibrin like necrosis, and the increase of chimerism.
2, Dysferlin was expressed in all groups of placenta, mainly distributed in the placental syncytial cell membrane. The expression of Dysferlin in the experimental group was lower than that of the control group, the severe preeclampsia group (2.07 + 1.83), and the mild preeclampsia group (4.67 + 1.95) compared with the group (6.64 + 2.14), there were significant differences (P0.05), pregnancy period. The hypertension group (5.60 + 1.45) had no significant difference compared with the control group (P0.05).
3, Caspase-3 was expressed in all groups of placenta, mainly distributed in the placental syncytial trophoblast pulp and part of the nucleus. The expression of Caspase-3 in the experimental group was higher than that of the control group, the severe preeclampsia group (7.20 + 1.74), the mild preeclampsia group (5.93 + 1.28) and the pregnancy induced hypertension group (5.33 + 1.45) and the control group (2 There were significant differences in.64 + 2.25 (P0.05).
4, there was a negative correlation between Dysferlin and Caspase-3 expression in placenta, and the correlation coefficient r=-0.4167 (P0.001).
Conclusion: 1, Dysferlin is expressed in the syncytial trophoblast membrane of placenta and weakened with the aggravation of the condition of hypertensive disorder complicating pregnancy.
2, the expression of Caspase-3 in the cytoplasm and nucleus of placental syncytial trophoblastic cells increased with the aggravation of the condition of hypertensive disorder complicating pregnancy.
3, the expression of Dysferlin and Caspase-3 in the placental chimeric trophoblast is negatively correlated, and the interaction between them may be related to the occurrence and development of hypertensive disorder in pregnancy.
