ING4、PHB与Ki-67在宫颈病变组织中的表达与HR-HPV感染的相关性研究

【摘要】:目的:探讨宫颈病变组织中的ING4、PHB、Ki-67的表达及其与HR-HPV感染的相关性。方法:选取2010年1月-2015年10月在我院经病理诊断为宫颈病变的132例的患者宫颈组织,分为慢性宫颈炎39例,宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)1级23例,CIN2-3级28例,宫颈鳞癌42例。采用免疫组织化学检测技术检测不同宫颈病变组织中生长抑制因子4(Inhibitor of growth family member 4,ING 4)、抗增殖蛋白(Prohibitin,PHB)、Ki-67的表达情况,采用杂交捕获二代(Hrbird Capture2,HC2)技术检测患者宫颈分泌物中高危型人乳头瘤病毒(high-risk human papillomavirus,HR-HPV)的感染情况,同时检测HPV-DNA的裁量。结果:1.ING4在正常宫颈上皮组织的细胞核中阳性表达。其在不同宫颈病变组织中表达不同,在慢性宫颈炎、CIN 1级、CIN 2-3级、宫颈鳞癌组中ING4表达的中强阳性表达率分别为:74.35%、60.87%、7.14%、54.76%,总体差异有统计学意义(Hc=34.805,P0.01),进一步两两比较显示,慢性宫颈炎组与CIN 2-3级组、慢性宫颈炎组与宫颈鳞癌组、CIN 1级组与宫颈鳞癌组、CIN 2-3级与宫颈鳞癌组的差异均有意义(P0.01)在慢性宫颈炎与宫颈癌前病变的三组中,ING4的表达与病变程度呈负相关(r=-0.228,P0.05),而在宫颈癌组织中,ING4表达较癌前病变组织异常升高。2.PHB在宫颈上皮组织中的细胞浆中阳性表达。在慢性宫颈炎、CIN 1级、CIN 2-3级、宫颈鳞癌组中PHB表达的强阳性率(+++)分别为35.9%、47.8%、57.1%和71.4%,总体差异有统计学意义(Hc=11.552,P0.01),且PHB的阳性表达程度与宫颈组织病变程度呈正相关(r=0.291,P0.01)。3.Ki-67的阳性表达主要定位于宫颈上皮组织的细胞核中,其在慢性宫颈炎、CIN 1级、CIN 2-3级、宫颈鳞癌组中Ki-67表达的阳性率分别为7.7%、73.9%、100%和100%,其中强阳性率(≥++)分别为2.6%、13%、75%和97.6%,总体差异有统计学意义(Hc=99.177,P0.01),Ki-67与宫颈病变的严重程度呈正相关(r=0.860,P0.01)。4.HR-HPV DNA的裁量与宫颈病变程度呈正相关(r=0.445,P0.01)。5.在宫颈组织中PHB、ING4与HR-HPV不相关。Ki-67与HR-HPV呈正相关(r=0.353,P0.01)。ING4与Ki-67的表达呈负相关(r=-0.256,P0.01),但PHB与Ki-67的表达之间呈正相关(r=0.369,P0.01),ING4与PHB的表达之间不相关。6.在宫颈癌组织中,ING4的表达与宫颈鳞癌组织的分化程度呈负相关(P0.05),组织分化程度越低,ING4在宫颈上皮组织中的表达越少,ING4的表达与宫颈癌患者的年龄、FIGO分期、肿瘤大小、浸润程度、淋巴结转移均无显著相关。PHB在宫颈鳞癌组的表达与年龄、分化程度、临床分期、肿瘤大小、浸润程度、淋巴结转移亦均无显著相关性。结论:1.PHB、ING4、Ki-67在宫颈上皮组织中的表达及HR-HPV的感染在宫颈癌的发生发展中有重要作用。2.ING4、PHB可能通过Ki-67与HR-HPV互相协同致癌或者抑癌。3.在宫颈癌中,ING4、PHB可能参与了宫颈癌的发生发展,有可能成为新的宫颈癌潜在的诊断标记物及治疗靶点。
[Abstract]:Objective: to investigate the expression of Ki-67 and its correlation with HR-HPV infection in cervical lesions. Methods: 132 cases of cervical lesions diagnosed pathologically in our hospital from January 2010 to October 2015 were divided into three groups: 39 cases of chronic cervicitis, 23 cases of cervical intraepithelial neoplasia (cervical intraepithelial plasia cin) grade 1, 28 cases of CIN2-3 grade, and 42 cases of cervical squamous cell carcinoma. Immunohistochemical technique was used to detect the expression of inhibitor of growth family member 4 and anti proliferating protein Ki-67 in different cervical lesions. The infection of high risk human papillomavirus (high-risk human papillomavirus) HR-HPV in cervical secretions was detected by hybrid capture 2 HC2 technique, and the discretion of HPV-DNA was detected at the same time. Results 1. ING4 was expressed in the nuclei of normal cervical epithelium. The positive rate of ING4 expression in cervical squamous cell carcinoma group was 74.35%, 60.87% and 7.14%, respectively. However, the expression of ING4 in cervical carcinoma was significantly higher than that in precancerous lesions. 2. PHB was positively expressed in the cytoplasm of cervical epithelium. In chronic cervicitis, CIN1, CIN2-3, The strong positive rate of PHB expression in cervical squamous cell carcinoma group was 35.9% and 71.4%, respectively. The overall difference was statistically significant (Hcn11.552, P0.01), and the positive expression of PHB was positively correlated with the degree of cervical lesions (rm0.291p0.01) .3.Ki-67 positive expression was mainly located in cervix.
