活血消异方抑制盆腔子宫内膜异位症保守术后复发的病例研究

【摘要】：目的:研究活血消异方抑制盆腔子宫内膜异位症保守术后复发的有效性和安全性。方法:选择符合诊断标准并签署知情同意书的子宫内膜异位症患者66例,给予活血消异方并于术后第1个月经周期第1-5天进行第1次访视,治疗期间每个月经周期访视1次至术后12月。术后3、6、9、12个月分别评价复发情况、妊娠情况、血清CA125水平及用药安全性。结果:纳入66例患者中,脱落6例,共60例纳入统计;术后6个月内可能是保守术后复发的高发期,以25-29岁复发数最多;活血消异方治疗盆腔子宫内膜异位症气滞血瘀证保守术后12个月内总复发率13.3%(8/60);单纯卵巢型子宫内膜异位症患者最多,术后12个月内总复发率12.0%(6/50),卵巢型子宫内膜异位症合并浸润型的术后12个月内总复发率11.1%(1/9),卵巢型子宫内膜异位症合并盆腔腹膜型的复发率最高,术后12个月内总复发率100%(1/1);r-AFS分期中Ⅲ、Ⅳ期患者的复发率较高,术后12个月内总复发率17.8%(8/45);60例病例中,有生育要求的患者26例,术后12个月内总妊娠率为61.5%(16/26);本方对病患术后3个月血清CA125水平有一定下调作用;仅有1例患者出现服药后不适,与患者药物性肝炎病史有关。结论:活血消异方对抑制盆腔子宫内膜异位症气滞血瘀证保守术后复发有效且安全,以单纯卵巢型子宫内膜异位症效果最佳。
[Abstract]:Objective: to study the efficacy and safety of Huoxue Xiaoyi decoction (HXXXY) in inhibiting the recurrence of pelvic endometriosis after conservative operation. Methods: 66 patients with endometriosis who met the diagnostic criteria and signed an informed consent were given Huoxue Xiaodiao recipe and the first visit was conducted on the 1st and 5th day of the menstrual cycle after operation. Visits were made once per menstrual cycle to 12 months after treatment. Recurrence, pregnancy, serum CA125 levels and drug safety were evaluated at 3, 6 and 12 months postoperatively. Results: among the 66 patients, 6 cases were dropped out, 60 cases were included in the statistics, the recurrence period may be high in 6 months after operation, and the number of recurrence in 25-29 years old is the most. The total recurrence rate was 13.3% (8 / 60) in 12 months after treatment of pelvic endometriosis with Qi stagnation and blood stasis syndrome. The total recurrence rate was 12.0% (6 / 50) in 12 months after operation, 11.1% (1 / 9) in ovarian endometriosis with infiltration, and the highest in ovarian endometriosis with pelvic peritoneal type.
