剖腹产子宫疤痕缺损的临床诊断和治疗

【摘要】：目的:探讨剖腹产子宫疤痕缺损的临床特征及诊断治疗方案。方法:纳入我院2013年6月-2014年10月期间收治的100例通过阴道B超确诊为子宫疤痕缺损,并经药物治疗无效的妇女,分别采用镜电切(50例)或阴式子宫疤痕修补术(50例)治疗,术后随访1~2年。结果:宫腔镜手术组治愈15例,好转20例,无效15例,总有效率为70.0%;阴式手术组仅有2例无效,治愈31例,好转17例,总有效率为96.0%。阴式手术组住院时间、住院费用比宫腔镜手术组明显增加(P0.05)。阴式手术组、宫腔镜手术组术后并发症发生率分别为8.0%、18.0%,两组比较,差异具有统计学意义(P0.05)。结论:阴式手术治疗剖宫产术后子宫疤痕缺损,微创,并发症少,效果令人满意。
[Abstract]:Objective: to investigate the clinical features and diagnosis and treatment of uterine scar defect in caesarean section. Methods: from June 2013 to October 2014, 100 women with uterine scar defect diagnosed by vaginal ultrasound were treated with electrosurgical resection (50 cases) or vaginal scar repair (50 cases), respectively. Postoperative follow-up was 1 ~ 2 years. Results: in hysteroscopic group, 15 cases were cured, 20 cases were improved, 15 cases were ineffective, the total effective rate was 70.0%, in the group of vaginal operation, only 2 cases were ineffective, 31 cases were cured, 17 cases were improved, the total effective rate was 96.0%. The length of hospitalization and the cost of hospitalization in the negative operation group were significantly higher than those in the hysteroscopic operation group (P0.05). The incidence of postoperative complications in the negative operation group and hysteroscopic operation group were 8.0 and 18.0, respectively. The difference between the two groups was statistically significant (P0.05). Conclusion: vaginal operation is effective in the treatment of uterine scar defect after cesarean section.
