双胎绒毛膜性与子痫前期发病风险的Meta分析

【摘要】：目的:明确绒毛膜性是否为双胎妊娠子痫前期(PE)的发病风险。方法:应用preeclampsia、eclampsia、hypertensive disorders、perinatal outcome、maternal pregnancy outcomes、chorionic、dichorionic、monochorionic等关键词检索Pub Med和Web of science等外文检索库,并通过妊娠期高血压疾病、子痫前期、子痫、绒毛膜性、单绒毛膜性、双绒毛膜性等检索关键词检索中国知网、维普、万方等中文检索库,检索截至2015年11月20日。按纳入和排除标准选取相关文献并提取相关数据。采用Rev Man5软件进行统计分析。结果:共纳入15篇文献,共10507例双胎妊娠,PE患者1371例,发病率13.05%。单绒毛患者2647例,其中PE患者329例,发病率12.43%;双绒毛患者7860例,其中PE患者1042例,发病率13.26%。Meta结果显示,欧洲组:OR=0.87,95%CI为0.60~1.28,P=0.02;亚洲组:OR=1.40,95%CI=0.82~2.38,P=0.47;北美洲组:OR=1.14,95%CI为0.75~1.75,P=0.004;3个亚组合并:OR=1.04,95%CI为0.80~1.36,P=0.0002。结论:欧洲组单绒毛膜性为双胎妊娠PE高危因素,北美洲组双绒毛膜性为双胎妊娠PE高危因素,亚洲组绒毛膜性不是双胎妊娠PE高危因素。
[Abstract]:Objective: to determine whether chorionic villus is the risk of preeclampsia (PE) in twin pregnancy. Methods: using preeclampsiaeclampsia1 (preeclampsia) perinatal disorder pregnancy outcomers monochorionic keywords, such as Pub Med and web of science, were used to search the foreign languages such as Pub Med, preeclampsia, eclampsia, chorionic membrane, monocaporionic membrane, eclampsia, preeclampsia, eclampsia, eclampsia. Double chorionic villi and other keywords to search the Chinese search database, such as China Zhiwang, Weipu, Wanfang, etc., as of Nov. 20, 2015. Select relevant documents and extract relevant data according to inclusion and exclusion criteria. The software Rev Man5 was used for statistical analysis. Results: a total of 10507 cases of PE with twin pregnancy were included in 15 articles, and the incidence rate was 13.05%. There were 2647 patients with single chorionic villi, including 329 patients with PE, and 7860 patients with double villi, including 1042 patients with PE, the incidence of which was 13.26.Meta. The results showed that the CI of the European group was 0.600.601.28p0.02; that of the Asian group was 0.822.388P0.47; that of the North American group was 0.751.1495 / 95 CI was 0.751.755; and that of the three subgroups was 0.801.39.95 CI was 0.801.395.The CI of the North American group was 0.751.795. Conclusion: single chorionic chorion is a high risk factor for PE in twin pregnancy in Europe, double chorion is a high risk factor for PE in twin pregnancy in North America, and chorion is not a high risk factor for PE in twin pregnancy in Asia.
