既往不良妊娠史ABO型血孕妇产前血清lgG抗体效价检测的临床意义

【摘要】：目的探讨不良妊娠史ABO型血孕妇产前检测血清Ig G抗体效价的临床意义。方法选取既往不良妊娠史孕妇92例(血清标本),其中母子ABO血型不合86例,Rh血型不合6例。分别检测孕妇血清中Ig G抗A(B)抗体效价及Ig G抗C(D)抗体效价,对Ig G抗体效价较高存在新生儿溶血病(HDN)发病风险孕妇进行孕期干预。结果 ABO血型不合86例,确诊HDN 52例,HDN发生率为60%。Ig G抗A(B)抗体效价1∶64孕妇11例,经菌陈汤治疗后均分娩健康婴儿;Ig G抗A(B)抗体效价≥1∶128孕妇77例,经吸氧、静脉滴注维生素C等治疗,59例胎儿出生并存活,但均存在不同程度黄疸,后经光疗、静滴丙种球蛋白治疗痊愈。Rh血型不合6例,孕妇Ig G抗D抗体阳性5例,抗体效价为1∶16;Ig G抗C抗体阳性1例(死胎)。5例Ig G抗D抗体阳性孕妇孕期行血浆去除及静滴丙种球蛋白治疗,1例胎儿出生2 h死亡,4例胎儿出生临床确诊为均Rh血型不合HDN行换血治疗治愈出院。结论既往不良妊娠史ABO型孕妇存在较高的HDN发病风险,产前检测血型lg G抗体效价并及时治疗干预对预防HDN的发生有重要意义。
[Abstract]:Objective to investigate the clinical significance of prenatal detection of serum IgG antibody in ABO type pregnant women with adverse pregnancy history. Methods 92 pregnant women (serum samples) with previous adverse pregnancy history were selected, including 86 cases of maternal and child ABO blood group incompatibility and 6 cases of Rh blood group incompatibility. The titer of IgG anti-A (B) antibody and IgG anti-C (D) antibody in pregnant women's serum were detected respectively. The pregnant women with high Ig G antibody titer were treated with high risk of neonatal hemolytic disease (HDN).
