GDM母血、脐血IGF-1水平与新生儿神经行为评分的相关性

【摘要】:目的1检测孕妇血清IGF-1、空腹血糖、空腹胰岛素及脐血IGF-1水平,分析IGF-1与妊娠期糖尿病胰岛素抵抗的关系;2分析GDM新生儿NABA评分与IGF-1、HOMA-IR等因素的关系,探讨影响妊娠期糖尿病新生儿神经行为发育的因素;方法随机选取2015年10月至2016年4月在华北理工大学附属医院定期孕检且分娩的妊娠期糖尿病孕妇47例作为GDM组,同时选取同期分娩的正常孕妇55例作为对照组。于分娩前抽取两组孕妇的空腹肘静脉血,于分娩时抽取两组孕妇胎儿的脐静脉血。应用酶联免疫(ELISA)法对两组孕妇及其脐血中的IGF-1进行测定,分别应用葡萄糖氧化酶法和电化学发光免疫分析法对孕妇空腹血糖和空腹胰岛素进行测定,并根据新生儿神经行为测定(NBNA)评分法评估新生儿神经系统发育的情况,胰岛素抵抗程度用胰岛素抵抗指数(HOMA-IR)来衡量。整理数据,计量资料采用t检验,计数资料采用c2检验,采用Pearson相关分析法判断两指标间的关系,一个因变量和多个自变量间的关系采用多元线性回归分析法。结果1 GDM组孕妇血清IGF-1(176.03±15.98pg/ml)低于对照组孕妇血清IGF-1(190.14±33.34pg/ml),且差异有统计学意义(t=-2.786,P=0.007);GDM组脐血IGF-1(181.85±16.49pg/ml)高于对照组脐血IGF-1(169.34±18.92pg/ml),差异有统计学意义(t=3.531,P=0.001);GDM孕妇血清IGF-1与其胎儿脐血IGF-1呈负相关(r=-0.323,P=0.027)。2 GDM孕妇的空腹血糖、空腹胰岛素、胰岛素抵抗指数明显高于对照组,差异有统计学意义(P0.01);GDM组孕妇血清IGF-1与HOMA-IR呈负相关(r=-0.446,P=0.002),GDM组脐血IGF-1与HOMA-IR呈正相关(r=0.586,P=0.000)。3 GDM组新生儿NABA评分(36.13±1.63分)低于对照组(37.49±1.48分),差异有统计学意义(t=-4.403,P=0.000)。4经Pesrson相关分析可得,妊娠期糖尿病组新生儿NABA评分与孕妇空腹血糖、空腹胰岛素、HOMA-IR、脐血IGF-1呈负相关关系(P0.05),与母血IGF-1呈正相关关系(P0.05);经多元线性回归分析可得,母血IGF-1、脐血IGF-1与新生儿NABA评分有密切关系,母儿血清IGF-1水平异常可能会影响妊娠期糖尿病新生儿神经行为发育情况。结论1妊娠期糖尿病孕妇血清中IGF-1水平显著降低,而其脐血中IGF-1水平显著升高;二者呈负相关。2妊娠期糖尿病孕妇血清IGF-1的低水平、脐血IGF-1的高水平与胰岛素抵抗有关,说明IGF-1可能参与了GDM的胰岛素抵抗。3妊娠期糖尿病孕妇新生儿神经行为评分明显降低,主要表现在行为能力、主动肌张力及被动肌张力方面;母血IGF-1降低、脐血IGF-1升高与妊娠期糖尿病新生儿神经行为发育异常关系密切。
[Abstract]:Objective 1 to detect the levels of serum IGF-1, fasting blood glucose, fasting insulin and IGF-1 in pregnant women, and to analyze the relationship between IGF-1 and insulin resistance in gestational diabetes mellitus (GDM). Methods 47 pregnant women with gestational diabetes mellitus (GDM) were randomly selected as GDM group from October 2015 to April 2016 in the affiliated hospital of North China University of Technology. At the same time, 55 normal pregnant women were selected as control group. Fasting cubitus vein blood was drawn from two groups of pregnant women before delivery and umbilical vein blood from fetus of two groups of pregnant women during delivery. The levels of IGF-1 in two groups of pregnant women and their umbilical blood were determined by enzyme linked immunosorbent assay (Elisa), and fasting blood glucose and fasting insulin were measured by glucose oxidase method and electrochemiluminescence immunoassay, respectively. The neonatal neurobehavioral test (NBNA) was used to evaluate the development of neonatal nervous system. The degree of insulin resistance was measured by the insulin resistance index (HOMA-IR).
