双胎妊娠胎儿游离DNA比例及其影响因素研究

【摘要】:目的探讨双胎妊娠母体血中胎儿游离DNA的比例及其与妊娠周数和孕妇体重指数的关系。方法研究对象为2015-10-01—2016-08-31在中国医科大学附属盛京医院产科就诊,自愿接受无创产前检测的妊娠11~30周双胎孕妇。通过后续随访,确认已有妊娠结局的男-男双胎71例,其中单绒毛膜双羊膜囊双胎31例,双绒毛膜双羊膜囊双胎40例。应用高通量测序技术,根据Y染色体的浓度估算胎儿游离DNA占孕妇血浆中总DNA的比例。结果双胎妊娠孕妇血浆中胎儿游离DNA比例5%~30%,并与孕周呈正相关(相关系数r=0.71),与孕妇体重指数呈负相关(相关系数r=-0.78)。结论在11~30周双胎妊娠母体血浆中,胎儿游离DNA的比例占总DNA的5%~30%,并随着孕周的增加而上升,随着孕妇的体重指数增加而下降。
[Abstract]:Objective to investigate the ratio of fetal free DNA in maternal blood of twin pregnancy and its relationship with gestational weeks and maternal body mass index (BMI). Methods the subjects of the study were pregnant women with 1130 weeks of gestation who voluntarily received non-invasive prenatal examination at Shengjing Hospital affiliated to China Medical University on May 10-01 to 2016-08-31. After follow-up, 71 cases of male twins with pregnancy outcome were confirmed, including 31 cases of single chorionic double amniotic sac twins and 40 cases of double chorionic double amniotic sac twins. High throughput sequencing technique was used to estimate the ratio of fetal free DNA to total DNA in maternal plasma according to the concentration of Y chromosome. Results the ratio of fetal free DNA in plasma of twin pregnant women was 30%, which was positively correlated with gestational weeks (r = 0.71) and negatively correlated with maternal body mass index (r = 0.78). Conclusion in the maternal plasma of 1130 weeks twin pregnancy, the ratio of fetal free DNA to total DNA is 5% and 30%, and increases with the increase of gestational week, and decreases with the increase of body mass index of pregnant women.
