早期经阴道穿刺放腹水治疗中重度卵巢过度刺激综合征的临床效果分析

【摘要】：目的:探讨早期经阴道穿刺放腹水治疗中重度卵巢过度刺激综合征(ovarian hyperstimulation syndrome,OHSS)的临床效果。方法:回顾性分析2014年1—9月在本院生殖医学中心行体外受精/卵胞浆内单精子注射-胚胎移植(IVF/ICSI-ET)术后出现中、重度OHSS的96例门诊患者临床资料。根据OHSS发生的时间分为早发型和晚发型。根据经阴道穿刺放腹水处理治疗的时间早晚,又进一步分为早处理组和晚处理组。结果:(1)中、重度OHSS患者早处理和晚处理组年龄、体重指数(BMI)、基础卵泡刺激素(FSH)等基本临床资料指标差异无统计学意义。其受精率、获卵数、流产率等指标差异也无统计学意义。(2)在两种OHSS分型患者中,早处理和晚处理组经阴道穿刺放腹水量差异均有统计学意义。结论:对于门诊管理的OHSS患者,无论是早发型还是晚发型,建议及早经阴道穿刺放腹水治疗,该治疗手段不增加流产率,降低了治疗后住院率,安全经济有效。
[Abstract]:Objective: to investigate the clinical effect of early vaginal puncture and ascites in the treatment of moderate and severe ovarian hyperstimulation syndrome (ovarian hyperstimulation syndrome). Methods: the clinical data of 96 outpatients with severe OHSS after in vitro fertilization / intracytoplasmic sperm injection-embryo transfer (IVF/ICSI-ET) in our hospital from January to September 2014 were analyzed retrospectively. According to the time OHSS occurs, it is divided into early and late hairstyles. According to the treatment time of vaginal puncture and ascites treatment, it was further divided into early treatment group and late treatment group. Results: (1) there was no significant difference in age, body mass index (BMI),) basic follicle stimulating hormone (FSH) and other basic clinical data between early and late treatment groups in patients with severe OHSS. There was no significant difference in fertilization rate, egg number, abortion rate and so on. (2) in the two OHSS typing patients, there were significant differences between the early treatment group and the late treatment group. Conclusion: for OHSS patients with early onset or late onset, it is recommended to treat ascites through vaginal puncture as early as possible. The treatment does not increase abortion rate, reduce the hospitalization rate after treatment, and is safe, economical and effective.
