肿瘤干细胞标记物CD133、Nestin、CD44和ALDH1A1在子宫内膜增生性病变和I型内膜癌中的表达

【摘要】:研究背景恶性肿瘤中存在肿瘤干细胞,这些细胞含量极少,具有自我更新、多向分化和体内致瘤的能力,并且与肿瘤的发生、发展、转移和复发密切相关。肿瘤干细胞有其标志物,不同的肿瘤其干细胞也不尽相同,迄今子宫内膜肿瘤干细胞缺乏特异性的表面标记物,其在子宫内膜癌的发生发展过程中的生物学功能也所知甚少。 目的本课题研究常见的肿瘤干细胞标记CD133、Nestin、CD44和ALDH1A1在子宫内膜增生性病变及I型子宫内膜癌中的表达变化,以探讨这些标志物在子宫内膜癌发生发展中的可能作用。 方法该研究收集2003年5月至2010年12月在上海交通大学医学院附属第九人民医院行子宫切除或诊断性刮宫的石蜡组织标本86例,其中正常增殖期子宫内膜15例(A组)、子宫内膜单纯及复杂性增生24例(B组)、子宫内膜不典型增生23例(C组)和I型子宫内膜癌24例(D组)。所有病例临床资料完整,病理诊断明确。然后采用组织芯片免疫组织化学技术检测CD133、Nestin、CD44和ALDH1A1四个蛋白从正常增殖期子宫内膜到子宫内膜癌变过程中的表达变化,并且分析其表达与子宫内膜癌的FIGO临床分期、组织学分级和肌层浸润深度等临床病理指标之间的关系。 结果CD133、CD44和ALDH1A1蛋白在上述4组中表达阳性率逐渐升高;而Nestin自A组至C组则表现为逐渐升高,在D组降低。组间比较显示CD133在C组和D组中的阳性表达均显著高于A组和B组(P<0.05),而C组与D组、A组与B组间的阳性表达无显著差异(P>0.05);CD44在D组中的阳性表达显著高于A组和B组(P<0.05),且CD44在B和C组中的阳性表达均显著高于A组(P<0.05),而C组与D组、B组与C组间的阳性表达则无显著差异(P>0.05);Nestin在B组和C组的阳性表达均显著高于A组和D组(P<0.05),而A组与D组、B组与C组间的阳性表达则无显著差异(P>0.05);ALDH1A1在D组中的阳性表达显著高于A组(P<0.05),其余各组之间的阳性表达差异不显著(P>0.05)。而且CD133的表达与子宫内膜癌的FIGO分期、组织分级和肌层浸润深度呈正相关,ALDH1A1的表达与子宫内膜癌的FIGO分期和肌层浸润深度呈正相关,与组织学分级不相关,,而CD44和Nestin的表达与临床病理指标间未见显著相关。 结论1.肿瘤干细胞标记物CD133、CD44和ALDH1A1在子宫内膜增生、不典型增生和癌组织中的高表达,提示其可能参与了子宫内膜癌的发生发展过程。 2. CD133和ALDH1A1与I型子宫内膜癌的预后有一定相关性,可作为子宫内膜癌预后的参考指标进一步研究;CD44与I型子宫内膜癌的预后因素无相关性。 3. Nestin在子宫内膜过度增生及不典型增生发生及进展过程中有一定的作用,但在癌症发生后其作用则有所减弱,还需要进一步的研究。
[Abstract]:Background there are tumor stem cells in malignant tumors, which have the ability of self-renewal, multi-differentiation and tumorigenesis in vivo, and are closely related to the occurrence, development, metastasis and recurrence of tumors. Tumor stem cells have their own markers, and different tumors have different stem cells. Up to now, endometrial cancer stem cells lack of specific surface markers, and their biological functions in the development of endometrial carcinoma are little known. Objective to investigate the expression changes of CD133nestinng CD44 and ALDH1A1 in endometrial hyperplasia and type I endometrial carcinoma in order to explore the possible role of these markers in the carcinogenesis and development of endometrial carcinoma. Methods from May 2003 to December 2010, 86 paraffin tissue specimens were collected from the ninth people's Hospital affiliated to Shanghai Jiaotong University Medical College, which underwent hysterectomy or diagnostic curettage. There were 15 cases of normal proliferative endometrium (group A), 24 cases of simple and complex hyperplasia of endometrium (group B), 23 cases of endometrial atypical hyperplasia (group C) and 24 cases of type I endometrial carcinoma (group D). All cases had complete clinical data and definite pathological diagnosis. Then the expression of CD133Nestinan CD44 and ALDH1A1 was detected by tissue microarray immunohistochemical technique in the process of endometrial carcinogenesis from normal proliferative phase to endometrial carcinogenesis.
