经阴道子宫输卵管动态三维超声造影评价输卵管通畅性的有效性和安全性

【摘要】:目的探讨经阴道子宫输卵管动态三维超声造影评价输卵管通畅性的有效性和安全性。方法对125例不孕患者,采用SonoVue造影剂,进行动态三维超声造影,记录相关指标,其中35例结果与腹腔镜通染液试验(CLP)进行对照,评价动态三维超声造影评估输卵管通畅性的有效性和安全性。结果 125例患者共计242条输卵管,超声造影结果显示输卵管通畅88条,输卵管通而不畅84条,输卵管阻塞70条。动态三维超声造影诊断输卵管阻塞的敏感度为92.00%、特异度93.33%、阳性预测值88.46%、阴性预测值95.45%、阳性似然比13.8,阴性似然比0.086。一致性分析显示,两种方法高度一致,Kappa值为0.846;125例患者造影过程中不同程度腹痛发生率为96%;术后阴道出血持续时间为(1.21±0.30)d,所有病例术后无阴道感染发生。结论动态三维超声造影评价输卵管通畅性具有较高的有效性和安全性,可作为临床评价输卵管通畅性的筛查方法之一。
[Abstract]:Objective to evaluate the efficacy and safety of transvaginal hysterosalpingography in the evaluation of tubal patency. Methods dynamic three dimensional contrast-enhanced ultrasonography was performed in 125 infertile patients with SonoVue contrast medium. The results of 35 cases were compared with that of Laparoscopic dye solution test (CLP). To evaluate the efficacy and safety of dynamic three-dimensional contrast-enhanced ultrasonography in evaluating the patency of fallopian tubes. Results there were 242 fallopian tubes in 125 patients, 88 tubal patency, 84 tubal patency and 70 tubal obstruction were found by contrast-enhanced ultrasonography. The sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio and negative likelihood ratio were 92.00, 93.33, 88.46, 95.4545, 13.8 and 0.086, respectively. Consistent analysis showed that the Kappa value of the two methods was 0.846% and the incidence of abdominal pain was 96, and the duration of postoperative vaginal bleeding was (1.21 卤0.30) days, and there was no vaginal infection in all cases. Conclusion dynamic three dimensional contrast-enhanced ultrasonography is effective and safe in evaluating the patency of fallopian tube, and it can be used as one of the screening methods for evaluating the patency of fallopian tube.
