高危型人乳头瘤病毒分型检测对宫颈癌及癌前病变筛查的意义

【摘要】：目的探讨高危型人乳头瘤病毒(HPV)分型检测对宫颈癌及癌前病变筛查的意义,为临床宫颈病变的诊治提供依据。方法选择2013年1月-2015年6月嘉善县第一人民医院妇科门诊就诊的并自愿进行HPV筛查的7 521例患者作为研究对象,所有研究对象均进行高危型HPV分型检测,对于HPV检测阳性或者有临床症状需要进行宫颈液基细胞学检查,HPV阳性或者宫颈液基细胞学检查阳性,或者临床症状需要行阴道镜检查者给予阴道镜检查,根据阴道镜检查情况,必要时行宫颈活检行病理学检查。观察HPV筛查患者中高危型HPV的感染情况,不同年龄段中HPV感染情况,不同宫颈病变中高危型HPV的感染情况。结果 7 521例进行HPV分型筛查的患者中,HPV阳性患者1 128例,HPV感染率为15.0%;HPV阳性患者的年龄高于HPV阴性患者(P0.05),不同高危分型HPV感染中,感染率最高的为HPV16型。HPV感染随着年龄的增加HPV感染率也不断增加,20~29岁年龄段患者HPV的感染率最低,为13.0%,60岁以上患者的HPV感染率最高,为36.9%,各年龄段之间HPV感染率之间比较,差异有统计学意义(P0.05)。325例行宫颈活检病理检查的患者中,检出CINⅠ115例、CINⅡ47例、CINⅢ80例、宫颈癌10例。不同宫颈病变中各种高危型HPV的感染情况之间比较,差异有统计学意义(P0.05),其中HPV16分型、HPV52分型、HPV56分型和HPV58分型宫颈病变发生的比例比较高。结论宫颈高危型HPV分型检测对宫颈癌及宫颈癌前病变的诊断具有重要意义,HPV16分型、HPV52分型、HPV56分型和HPV58分型宫颈癌和宫颈癌前病变的发生率比较高,在有条件的医院,宫颈高危型HPV分型检查可以作用宫颈癌和宫颈癌前病变的筛查手段。
