维生素D对妊娠糖尿病患者胰岛素敏感性及脂肪因子APN、RPB4、Visfatin及TNF-α的作用机制研究

【摘要】:目的:探讨血清维生素D水平与妊娠糖尿病(GDM)患者胰岛素敏感性及对脂肪因子脂联素(APN)、视黄醇结合蛋白4(RBP4)、内脂素(Visfatin)及肿瘤坏死因子(TNF-α)的影响及作用机制。方法:选择101例妊娠糖尿病患者(GDM组)和50例血糖正常孕妇(对照组),检测二组检测对象相关生化指标、APN、RBP4、Visfatin、TNFα及25-(OH)D3,之后在诊断为低维生素D患者中分别随机取12例GDM治疗组及12例控制组,补充1月维生素D再行相关生化指标检查。结果:GDM组患者测定25-(OH)D3及APN水平明显低于对照组(P<0.05),而RBP4、Visfatin、TNF-α水平高于对照组(均P<0.05);经补充维生素D约1月,,胰岛素分泌指数上升,胰岛素抵抗指数下降,而APN水平升高,与25-(OH)D3呈正相关(r值0.526); RBP4、Visfatin、TNF-α降低,与25-(OH)D3呈负相关(r值分别为-0.272,-0.153,-0.072)。结论:维生素D可通过降低相关脂肪因子RBP4、Visfatin、TNF-α,升高APN,从而对妊娠糖尿病起到保护作用。
[Abstract]:Objective: to investigate the relationship between serum vitamin D level, insulin sensitivity and adiponectin (APN), retinol binding protein 4 (RBP4) in patients with gestational diabetes mellitus (GDM). The effect and mechanism of (Visfatin) and TNF- 伪. Methods: 101 patients with gestational diabetes mellitus (GDM group) and 50 normal pregnant women with normal blood glucose (control group) were selected. The biochemical indexes, APN,RBP4,Visfatin,TNF 伪 and 25 (OH) D3 were detected in the two groups. Then 12 cases of GDM treatment group and 12 cases of control group were randomly selected in the diagnosis of low vitamin D. Results: the levels of 25- (OH) D3 and APN in GDM group were significantly lower than those in control group (P < 0. 05), while RBP4,Visfatin,TNF- 伪 level was higher than that in control group (all P < 0. 05). After vitamin D supplementation for about one month, insulin secretion index increased, insulin resistance index decreased, and APN level increased, which was positively correlated with 25- (OH) D3 (r = 0.526). The decrease of RBP4,Visfatin,TNF- 伪 was negatively correlated with 25 (OH) D3 (r = -0.272 ~ -0.153 ~ 0. 072). Conclusion: vitamin D can protect gestational diabetes by decreasing RBP4,Visfatin,TNF- 伪 and increasing APN,.
