针刺联合克罗米芬治疗黄体功能不足性不孕症临床观察

【摘要】：目的：本文通过对针刺联合克罗米芬治疗黄体功能不足性不孕症进行临床疗效观察,并与单纯克罗米芬组予以对照并且进行客观评价,积极探索针刺在治疗本病中的作用及机理,明确针刺具有改善卵巢功能,促进卵泡发育,提高黄体功能的重要作用。为治疗因黄体功能不足所引起的不孕症提出安全有效的临床治疗方案。方法：按照临床研究程序和方法,搜集符合纳入标准的因黄体功能不足导致不孕的患者36例,随机分为两组：治疗组18例应用针刺联合克罗米芬治疗,对照组18例单纯应用克罗米芬进行治疗。观察两组患者经过三个月经周期治疗前后的妊娠情况、中医证候、卵泡发育、子宫内膜情况、BBT、血清雌孕激素含量的改善情况及BMI的变化情况。经疗效评分与统计学方法处理,客观的评价针刺联合克罗米芬治疗本病的临床疗效。结果：经过针刺联合药物治疗后黄体功能不足性不孕患者有7例妊娠,妊娠率为41.18%,西药组5例妊娠,妊娠率为29.41%,比较无统计学意义(P0.05),但治疗组优于对照组；两组间比较治疗组在改善中医临床症状和基础体温的情况上明显优于对照组(P0.05)。在改善血清雌、孕激素、卵泡最大直径、子宫内膜厚度的对比中治疗组与对照组都具有统计学意义(P0.05),治疗组明显好于对照组。在体重指数的对照中,两组的比较无明显差异(P0.05)。结论：针刺具有改善卵巢的黄体功能,促进卵泡的发育,调整月经周期和提高血清雌、孕激素含量,增加妊娠率和降低早期流产率的作用,安全有效,患者无不良反应,值得进一步研究推广。
[Abstract]:Objective: To observe the effect and mechanism of acupuncture combined with Kuromifene in the treatment of infertility due to the deficiency of the corpus luteum, and to compare with the simple Kuromifene group and to carry out the objective evaluation, and to explore the role and mechanism of acupuncture in the treatment of the disease. It has an important role in improving the ovarian function, promoting the development of the follicle, and improving the function of the corpus luteum. And provides a safe and effective clinical treatment scheme for treating the infertility caused by the insufficiency of the corpus luteum function. Methods: According to the clinical study procedure and method,36 patients with infertility due to the deficiency of the corpus luteum were collected and randomly divided into two groups. The status of pregnancy, the syndrome of TCM, the development of the follicle, the condition of the endometrium, the BBT, the level of the estrogen and the progestogen in the serum and the change of the BMI were observed.
