Dyrk1B、IKKε、Lamininγ1在上皮性卵巢癌中的表达及意义

【摘要】:目的:初步评估上皮性卵巢癌的二元分型在临床应用的价值。检测P53、Ki-67、Dyrk1B、IKKε、Lamininγ1蛋白在上皮性卵巢癌型和型中表达的差异,发现新的区分两类卵巢癌的分子特征,探索临床治疗此病新的靶基因。 方法:回顾性分析2006年01月~2013年04月间的上皮性卵巢癌146例,按照二元论模型理论将其分为型和型,比较两组的临床参数及P53、Ki-67、Dyrk1B、IKKε、Lamininγ1的表达情况。 结果:上皮性卵巢癌146例,其中型61例,包括低级别浆液性癌11例、低级别子宫内膜样癌5例、粘液性癌29例、透明细胞癌15例;II型85例,包括高级别浆液性癌74例,高级别子宫内膜样癌11例。型患者平均年龄54.2岁,低于II型患者的平均年龄(59.7岁)(p<0.05)。型组中早期(Ⅰ-Ⅱ期)患者占78.7%,而型组的晚期(Ⅲ-Ⅳ期)患者则占(P<0.05)。型组术前血清CA125水平明显低于II型组(P<0.05)。型组中85.2%的患者肿瘤累及一侧卵巢,而型组中52.9%的患者肿瘤累及一侧卵巢,47.1%累及双侧卵巢,二者比较有显著差异(P<0.05)。型组的肿瘤体积大于型组(P<0.05)。型组的中位生存时间为67.8月,大于型组的中位生存时间(45.0月)(P<0.05)。对I和II型上皮性卵巢癌的多分子免疫组化检测发现,,Dyrk1B在I型组中高表达,而P53、Ki-67、Lamininγ1则在II型组中高表达(P<0.05),但IKKε在两类分型中无显著差异(p>0.05)。进一步分析免疫组化结果与生存时间的关系,发现P53、Dyrk1B、IKKε阳性组的术后生存时间均低于阴性组(P<0.05),而Ki-67、Lamininγ1阳性组与阴性组的生存时间无统计学差异(P>0.05)。 结论:I型上皮性卵巢癌患者与II型上皮性卵巢癌患者相比,更加年轻,分期更早,血清CA125水平偏低,但可形成更大的术前病灶,且术后生存时间更长。除IKKε外,P53、Ki-67、Dyrk1B、Lamininγ1蛋白在I、II型上皮性卵巢癌中表达具有显著差异,可作为辅助区分两类卵巢癌的分子特征。P53、Dyrk1B、IKKε的免疫组化检测对上皮性卵巢癌预后判断具有一定的指导作用。
