羊水栓塞孕兔血浆组织因子水平与凝血功能障碍的关系

【摘要】:目的探讨羊水栓塞(AFE)孕兔血浆组织因子(TF)水平与凝血功能障碍的关系。方法将30只健康孕晚期家兔随机分为A、B、C组各10只,分别经耳缘静脉输入1%羊水胎粪混合液、自体羊水2.5 mL/kg及等量生理盐水。分别于羊水注射前及注射后30 min,耳缘静脉取血检测血浆TF水平及凝血指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)及纤维蛋白原(FIB)]。结果各组注射前血浆TF及凝血功能指标比较差异无统计学意义(P均0.05),A、B组注射后较同组注射前及C组注射后血浆TF水平升高,PT、APTT、TT延长而FIB减少(P均0.05),且A组各指标变化大于B组(P均0.05)。血浆TF与PT、APTT、TT呈正相关(r分别为0.95、0.86、0.89,P均0.05),与FIB呈负相关(r=-0.92,P0.05)。结论 AFE发生时羊水中的TF大量进入母体血液循环,从而激活外源性凝血途径,引起母体凝血功能障碍。
[Abstract]:Objective to investigate the relationship between plasma tissue factor (TF) level and coagulation dysfunction in pregnant rabbits with amniotic fluid embolism (AFE). Methods 30 healthy rabbits in the third trimester of pregnancy were randomly divided into group A (n = 10) and group C (n = 10). 1% amniotic fluid meconium mixture, autologous amniotic fluid 2.5 mL/kg and the same amount of saline were infused into the ear margin vein, respectively. Plasma TF level and coagulation index were measured before amniotic fluid injection and 30 min, after amniotic fluid injection [prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT) and fibrinogen (FIB)]. Results there was no significant difference in plasma TF and coagulation function between the two groups before injection, but there was no significant difference in plasma TF and coagulation function between the two groups before injection. The plasma TF level and PT,APTT, in group B were higher than those before injection and group C after injection. TT prolonged and FIB decreased, and the changes of each index in group A were higher than those in group B (P < 0.05).
