miR-21、miR-27a、miR-27b在肥胖及代谢紊乱型多囊卵巢综合征患者淋巴细胞中的表达

【摘要】:背景:多囊卵巢综合征(polycystic ovary syndrome,PCOS)是一种常见的内分泌、代谢紊乱疾病,多伴有肥胖表型。其发病机制尚不明确,当前认为是由遗传、环境等多因素多环节共同引起。Micro RNA(miRNA)作为一种非编码小分子单链RNA,主要在转录后水平调控基因的表达。miRNA的异常表达与多种代谢紊乱有关,包括肥胖、糖尿病,以及PCOS。近来许多学者发现miR-21、miR-27a、miR-27b与炎症、激素代谢、脂肪合成、肥胖、胰岛素信号传导有关,然而目前关于miR-21、miR-27a、miR-27b的表达与PCOS患者肥胖及相关代谢紊乱的关系研究甚少。目的:研究肥胖和非肥胖型PCOS患者外周血淋巴细胞miR-21、miR-27a、miR-27b的表达水平及控制体重对其表达的影响,探讨miR-21、miR-27a、miR-27b与PCOS肥胖及相关代谢紊乱(高雄激素血症、高胰岛素血症)的关系。方法:1.纳入PCOS患者60例,根据WHO对成年人体重指数(BMI)的划分标准分为非肥胖组(BMI25kg/m2,22例)和肥胖组(BMI≥25kg/m2,38例)。2.采用电化学发光检测PCOS患者血清促卵泡素(b FSH)、促黄体生成素(b LH)、雌二醇(b E2)、孕激素(b P)、睾酮(bT)、空腹胰岛素(FINS)水平。3.采用葡萄糖氧化酶法测定PCOS患者空腹血糖(Glu)。4.应用实时荧光定量PCR(q PCR)法检测外周血淋巴细胞miR-21、miR-27a、miR-27b的表达水平。5.肥胖组控制体重后复测上述指标。结果:1.肥胖组PCOS患者bT、Glu、FINS、HOMA-IR水平均显著高于非肥胖组,且2.差异有统计学意义(P0.05)。3.肥胖组PCOS患者外周血淋巴细胞miR-21、miR-27a、miR-27b的表达水平明显高于非肥胖组(2.83±0.60 vs 1.18±0.24;2.63±0.42 vs 1.13±0.19;2.74±0.49 vs1.16±0.17;P均0.05)。4.配对样本t检验结果显示肥胖组PCOS患者控制体重后b FSH、b E2、b P的水平与控制体重前相比,差异无明显统计学意义(P0.05);BMI、b LH、bT、Glu、FINS、HOMA-IR水平较控制体重前显著降低,且差异有统计学意义(P0.05)。5.肥胖组PCOS患者控制体重后外周血淋巴细胞miR-21、miR-27a、miR-27b的表达水平较控制前明显降低(1.88±0.27 vs 2.81±0.61;2.16±0.39 vs 2.69±0.37;2.27±0.40 vs 2.72±0.47;P均0.05)。6.miR-21的表达量与bT呈正相关性(r=0.342,P=0.008),miR-27a、miR-27b的表达量与FINS呈正相关性(r=0.413,P=0.001;r=0.397,P=0.002)。结论:miR-21、miR-27a、miR-27b与PCOS肥胖及相关代谢紊乱(高雄激素血症、高胰岛素血症)息息相关。miR-21可能主要与bT代谢异常有关,而miR-27a、miR-27b可能主要与胰岛素代谢异常有关。miR-21、miR-27a、miR-27b分别通过不同途径参与了PCOS肥胖和高雄激素血症、高胰岛素血症的恶性循环。
