肥胖孕妇血脂水平与DNA损伤及总抗氧化能力水平的相关性

【摘要】:背景及目的:在女性妊娠期间,因为雌激素的作用,并且母体满足自身营养需求同时为满足胎儿生长发育的需要,女性体内的脂质包括TC、TG、LDL的含量会出现生理性的增高,而在肥胖孕妇其升高更为明显[1]。See-Ling Loy[2]等人的研究结果显示,在肥胖、脂代谢紊乱的双重作用之下,机体的氧化应激水平明显升高,自由基尤其活性氧簇过度产生,而过多的活性氧簇可以致使DNA链出现断裂、位点突变、双链畸变等病变,造成不同程度的DNA损伤[3]。现有的研究表明,孕妇体内氧化应激水平增高及其导致的DNA损伤和自然流产、胎膜早破和胎儿宫内窘迫等产科以及新生儿疾病关系密切[4],然而当前国内外对妊娠期间血清脂质水平和DNA损伤、总抗氧化能力的相关性研究缺乏[2,5]。故研究孕妇血脂与DNA损伤及总抗氧化水平的相关性,可能通过干预血脂水平等方面起到预防妊娠期不良事件的作用。方法:选取2011年9月到2012年9月在本院门诊做系统产检的孕妇120人纳入本次研究当中,按照体重指数分成肥胖组(n=75)和体重正常组(n=45)。分析并且比较两组TC、TG、LDL、HDL含量和DNA损伤情况以及总抗氧化能力水平。分析肥胖组血脂含量和DNA损伤、总抗氧化能力的相关性。结果:肥胖组TC、TG、LDL水平均较正常组显著增高(P=0.000,P=0.000,P=0.004);HDL水平显著则低于体重正常组(P=0.006)。肥胖组彗星实验尾力矩明显高于体重正常组(P=0.000),并且总抗氧化能力明显高于体重正常组(P=0.000)。DNA损伤与TC、TG、LDL水平呈显著正相关关系,(r=0.23,P=0.026;r=0.26,P=0.008;r=0.19,P=0.032);总抗氧化能力和TG含量呈显著正相关关系(r=0.32,P=0.000)。结论:肥胖孕妇在孕期常常出现脂代谢紊乱和体内氧化及抗氧化水平衡失调;DNA损伤和血清TC、TG、LDL含量呈显著正相关,总抗氧化能力与TG含量呈显著正相关。
