多囊卵巢综合征患者血清中脑星形胶质细胞源性神经营养因子水平及其与胰岛素抵抗的关系

目的检测多囊卵巢综合征(polycystic ovary syndrome,PCOS)患者血清中脑星形胶质细胞源性神经营养因子(mesencephalic astrocyte derived neurotrophic factor,MANF)的水平,探讨PCOS患者血清MANF水平与胰岛素抵抗、生殖内分泌激素及糖脂代谢指标之间的关系,评估二甲双胍治疗对PCOS患者血清MANF的影响。方法本研究纳入66例初诊PCOS患者和40例年龄匹配的健康成年女性。检测受试者糖化血红蛋白、血脂、性激素及血清MANF等。采用正葡萄糖-高胰岛素钳夹(euglycemic-hyperinsulinemic clamps,EHC)模型评估胰岛素抵抗程度,口服葡萄糖耐量试验(oral glucose tolerance test,OGTT)评估胰岛β细胞功能。采用Pearson相关和多元逐步线性回归分析血清MANF与胰岛素抵抗、生殖内分泌激素及其他糖脂代谢指标之间的关系。结果PCOS患者血清MANF水平低于健康对照组。相关分析提示,血清MANF水平与M值呈正相关,与稳态模型评估的胰岛素抵抗指数(homeostasis model assessment of insulin resistance,HOMA-IR)、OGTT中胰岛素曲线下面积(area under the curve of insulin,AUCi)/葡萄糖曲线下面积(area under the curve of glucose,AUCg)比值(AUCi/AUCg)、体质指数(body mass index,BMI)、腰臀比(waist to hip ratio,WHR)、体脂含量(body fat percentage,FAT)、黄体生成素/卵泡刺激素比值(luteinizing hormone/follicle stimulating hormone,LH/FSH)、游离睾酮指数(free androgen index,FAI)、非高密度脂蛋白胆固醇(non-high density lipoprotein cholesterol,non-HDL-C)及游离脂肪酸(free fatty acid,FFA)呈负相关。多元逐步线性回归分析提示血清MANF水平与M值、FAI独立相关。二甲双胍治疗后,PCOS患者胰岛素抵抗和高雄激素血症有所改善,血清MANF水平较治疗前显著升高。结论PCOS患者血清MANF水平是下降的,且与胰岛素抵抗呈负相关。血清MANF可能参与PCOS患者胰岛素抵抗发生、发展过程。
