不同梗死部位急性脑梗死患者血浆D二聚体水平变化及其与神经功能缺损程度的关系

【摘要】:目的观察不用梗死部位急性脑梗死患者血浆D二聚体水平变化,并分析其与神经功能缺损程度的关系。方法急性脑梗死患者150例,其中梗死部位位于小脑、脑干、基底节、大脑皮层、多部位者各30例(分别计为小脑组、脑干组、基底节组、大脑皮层组、多部位组),比较各组血浆D二聚体水平、左室射血分数(LVEF)及入院第1、10天NIHSS评分。结果与多部位组比较,其余4组血浆D二聚体水平降低;与小脑组及基底节组比较,脑干组、大脑皮层组血浆D二聚体水平升高;与大脑皮层组比较,其余4组LVEF增加;P均0.05。与小脑组比较,其余4组入院第1、10天NIHSS评分增加,神经功能缺损改善程度明显;与多部位组比较,其余4组入院第1、10天NIHSS评分降低;与大脑皮层组比较,脑干组入院第1天NIHSS评分增加,神经功能缺损改善程度明显;P均0.05。Spearman相关分析显示,D二聚体水平与入院第1、10天NIHSS评分呈正相关(r分别为0.395、0.414,P均0.05)。结论不同梗死部位急性脑梗死患者血浆D二聚体水平不同,其与神经功能缺损程度相关;此指标可作为病情及病变部位诊断的参考依据,亦可作为患者神经功能预后的参考指标。
[Abstract]:Objective to observe the changes of plasma D-dimer level in patients with acute cerebral infarction without infarction, and to analyze the relationship between D-dimer and the degree of neurological deficit. Methods 150 patients with acute cerebral infarction were treated with acute cerebral infarction, including cerebellum, brainstem, basal ganglia. There were 30 cases of cerebral cortex and 30 cases of multiple sites (cerebellar group, brainstem group, basal ganglia group, cerebral cortex group, multisite group). Left ventricular ejection fraction (LVEF) and NIHSS score on the 1st day after admission. Results compared with the multisite group, the plasma D-dimer levels in the other 4 groups were lower, and those in the brainstem group and cerebral cortex group were higher than those in the cerebellar group and basal ganglia group. Compared with the cerebral cortex group, the other four groups increased LVEF (P 0.05). Compared with the cerebellar group, the NIHSS score of the other four groups increased on the 1st day after admission, and the nerve function defect improved significantly, and the NIHSS score of the other four groups decreased on the 1st day after admission compared with the multi-site group. Compared with the cerebral cortex group, the NIHSS score of the brainstem group increased on the first day of admission. The degree of improvement of neurological function defect was significantly improved (P < 0.05). Spearman correlation analysis showed that there was a positive correlation between D dimer level and NIHSS score on the 1st day of admission, respectively. Conclusion the plasma D dimer levels are different in patients with acute cerebral infarction at different infarct sites. This index can be used as a reference for the diagnosis of the disease and the location of the lesion, as well as as a reference index for the prognosis of the neurological function of the patients.
