大鼠二次脑损伤合并海水浸泡后脑组织SOD、MDA变化的实验研究

【摘要】:研究背景及目的:二次脑损伤(secondary brain insults,SBI)常继发性发生创伤性脑损伤后数小时至数天,可进一步加重脑损伤。海上环境的特殊性及救治的延误,使得在SBI后易合并海水浸泡,其伤情更加复杂。本实验在SBI的模型基础上,建立大鼠SBI合并海水浸泡模型,观察SBI合并海水浸泡后脑组织超氧化物歧化酶(superoxide dismutase,SOD)、丙二醛(malondialdehyde,MDA)的变化规律,探讨二者在SBI合并海水浸泡发病机制的作用。 方法:163只雄性SD大鼠被随机分为正常对照(A组,5只)、SBI(B组,32只)、SBI合并生理盐水浸泡(C组,33只)、SBI合并海水浸泡(D组,93只)四组。在Marmarou弥漫性脑损伤(diffusion brain injury,DBI)动物模型基础上制作缺血性SBI合并海水浸泡动物模型,DBI稳定30min后结扎大鼠右侧颈总动脉,牙科钻钻开右侧颅骨,剪开硬脑膜,脑棉片覆盖并用输液器持续灌注海水(室温23℃),缝合头部皮肤,使局部脑组织持续浸泡在海水中。在特定时间(1h,3h,6h,12h,24h,48h)通过大脑HE染色比较海水浸泡与否对SBI脑水肿的影响。并对伤侧脑组织使用黄嘌呤氧化酶法测定SOD,硫代巴比妥酸法测定MDA。 结果:成功建立SBI动物模型,并在此基础上建立SBI合并海水浸泡模型,通过剪破硬脑膜,伤侧脑组织能够得到充分的海水浸泡,这一动物模型具备SBI及海水浸泡伤特点,满足本实验要求。SBI合并海水浸泡组大鼠死亡率(45/93,48.93%)远高于B组死亡率(8/32,25.00%)、C组死亡率(9/33,27.27%),有差异性(P<0.05)。SBI合并海水浸泡组出现脑水肿早于另外两组,SBI组及SBI合并生理盐水浸泡组脑组织含水量在模型制作完成后24h趋于稳定,SBI合并海水浸泡组持续性升高至48h(88.38±0.93%),脑组织含水量有差异(P<0.05),HE染色对SBI病理学改变观察比较结果表明海水浸泡能在早期加重脑水肿,SBI合并海水浸泡组后期损伤更严重,呈网格状改变,,单纯SBI、SBI合并生理盐水浸泡组脑组织呈海绵状改变。SBI合并海水浸泡组3h大鼠脑组织SOD含量(212.09±22.29)U/mgprot就有大幅降低,低于单纯SBI组3h含量(245.31±21.26)U/mgprot;SBI合并海水浸泡组3h大鼠脑组织MDA含量(9.53±0.47)nmol/mgprot明显升高,且高于单纯SBI组3h含量(6.16±0.34)nmol/mgprot。实验组大鼠脑组织中SOD含量与MDA含量成负相关,相关系数有差异性(P<0.05)。B组、C组、D组脑组织MDA含量与脑组织含水量成平行关系。 结论:①对应时间窗下,SBI合并海水浸泡比单纯SBI严重;②SBI合并海水浸泡伤程度发展更迅速;③SBI合并海水浸泡后自由基改变与脑损伤关系密切。
[Abstract]:Background and objective: secondary brain insults (SBI) can further aggravate the brain injury after secondary traumatic brain injury. The particularity of marine environment and the delay of treatment make it easy to combine seawater immersion after SBI. On the basis of the model of SBI, the rat model of SBI combined with seawater immersion was established to observe the changes of superoxide dismutase (SOD) and malondialdehyde (malondialdehyde) in brain tissue after SBI and seawater immersion. To explore the role of both in the pathogenesis of SBI combined with seawater immersion. Methods Sixty-three male Sprague-Dawley rats were randomly divided into four groups: normal control group A (n = 5), group B (n = 32) and group C (n = 33), group C (n = 33) and group D (n = 93). The animal model of ischemic SBI combined with seawater immersion was made on the basis of which the right common carotid artery was ligated after 30 minutes of stabilization.
