肝素化选择性羊ADM促进深二度烧伤创面愈合及对创面组织中成纤维细胞凋亡的影响

【摘要】:目的1.制作肝素化选择性羊脱细胞真皮基质(acelluar dermal matrix, ADM)。2.运用肝素化羊ADM治疗大鼠深二度烫伤,研究烫伤创面愈合率和创面组织结构的变化情况。3.探讨肝素化羊ADM治疗烧伤创面时,创面组织中细胞凋亡的变化情况,从而揭示该生物敷料在促进创面愈合的一些机制。方法1.选择1岁龄健康绵羊,取断层皮片制作选择性羊ADM。2.160只健康Wistar大鼠制作成深二度烫伤模型,随机分为碘伏纱布治疗组、猪ADM治疗组、羊ADM治疗组和肝素化羊ADM治疗组。3.烫伤大鼠在术3d、7d、14d、21d和28d计算大鼠创面愈合率。并同时在各个时间段处死大鼠,取创面组织标本做HE染色和TUNEL原位细胞凋亡染色,观察创面愈合不同时期内组织结构变化情况。结果1.成功制作选择性羊ADM,表皮完整,真皮未见细胞成分、血管及皮肤附件。2.烫伤后14d、21d、28d时,猪ADM组、羊ADM组和肝素化羊ADM组的创面愈合率均优于碘伏纱布组(P0.05),21d肝素化羊ADM组优于猪ADM组(P0.05),21d和28d时肝素化羊ADM组优于羊ADM组(P0.05)。3.各组创面HE染色显示,伤后21d,猪ADM组和羊ADM组真皮层胶原纤维排列欠规整,成纤维细胞开始出现较多的增长。而肝素化羊ADM组伤后21天时表皮、真皮结构基本正常,胶原纤维排列比较规则。4.各组深二度在烫伤后第7d,各组创面组织中成纤维细胞的凋亡较3d时均有明显的增加,但各组间无明显的统计差异(P0.05)。随着大鼠创面的愈合,在烫伤后14d-28d,各组创面组织中的成纤维细胞凋亡数有明显的增加,猪ADM组、羊ADM组和肝素化羊ADM组中的细胞凋亡数均较碘伏纱布组有明显的增加(P0.05),其中尤以肝素化羊ADM治疗组在术后28d的数字最高(P0.05)。结论1.成功制作了选择性羊皮真皮基质。2.三种真皮基质治疗深二度烧伤创面,其中尤以肝素化羊ADM在促进创面愈合组织结构恢复方面的效果更好。3.肝素化羊ADM在烧伤创面愈合后期,较其它组能更加的诱导或促进创面肉芽组织中成纤维细胞的细胞凋亡,减少瘢痕的形成,从而促进创面修复愈合。
[Abstract]:Objective 1. To make heparinized selective acelluar dermal matrix, ADM.2.Heparinized sheep ADM was used to treat deep second degree scalding in rats. The healing rate and tissue structure of burn wounds were studied. 3. To investigate the effect of heparin-treated sheep ADM on burn wounds. The changes of cell apoptosis in wound tissue, so as to reveal the mechanism of the biological dressing in promoting wound healing. 1. Choose 1 year old healthy sheep. The model of deep second degree scalding was made from selective sheep ADM.2.160 rats by skin grafts. The rats were randomly divided into two groups: iodophor gauze treatment group and porcine ADM treatment group. Sheep ADM group and heparinized sheep ADM group .3.The wound healing rate of scalded rats was calculated on the 3rd day, 7d, 14d, 21d and 28d, and the rats were killed at the same time. The wound tissue samples were stained by HE and TUNEL in situ apoptosis staining. The changes of tissue structure in different periods of wound healing were observed. 1. Selective sheep ADM was made successfully. The epidermis was intact, no cell components were found in dermis, blood vessels and skin appendages were found. At 14 days after burn, 21 days after scald, pig ADM group was treated. The wound healing rate of sheep ADM group and heparin treated sheep ADM group was better than that of iodophor gauze group on 21d after heparin treatment, and that of heparin treated sheep ADM group was better than that of porcine ADM group at 21d and 28d. The wound surface of heparin treated sheep ADM group was superior to that of ADM group (P 0.05N. 3). On the 21st day after injury, the collagenous fibers in the dermis of pig ADM group and sheep ADM group were irregular, and fibroblasts began to increase, while the epidermis and dermis structure of heparinized sheep ADM group were basically normal at 21 days after injury. On the 7th day after scalding, the apoptosis of fibroblasts in the wound tissue of each group increased significantly, but there was no significant statistical difference between the groups (P 0.05). With the healing of the wound in rats, the apoptosis of fibroblasts in the wound tissue of each group was significantly increased than that of the third day after scald, but there was no significant statistical difference between the groups. The number of fibroblast apoptosis in wound tissue of each group increased significantly at 14 d to 28 d after scalding, and the number of fibroblasts in pig ADM group was significantly increased. The number of apoptosis in sheep ADM group and heparinized sheep ADM group was significantly higher than that in iodophor gauze group, especially in heparinized sheep ADM group. Conclusion 1. Selective dermal matrix of sheep skin was successfully made. Treatment of deep second degree burn wounds with three kinds of dermal matrix, Especially heparinized sheep ADM was more effective in promoting the recovery of wound healing tissue structure. Heparin sheep ADM could induce or promote the apoptosis of fibroblasts in granulation tissue more than other groups in the late stage of burn wound healing. Reduce scar formation, thereby promoting wound healing.
