脓毒症患者血压变异率特点及临床意义

【摘要】:目的:观察脓毒症患者血压变异率的变化特征,研究其与脓毒症患者疾病危重程度关系;观察脓毒症患者血压变异率和内分泌、炎症指标(TNF-α、IL-10、 PCT)的相关性,探讨其可能机制。 方法:选取2011年11月--2012年4月期间入住天津医科大学总医院急救中心脓毒症患者89例,记录患者入院24小时内的一般临床指标、生命体征、血常规、肝功能、肾功能、PCT、电解质、血气分析、CRF、ESR、COR、ACTH、游离甲功,收集最差临床指标计算脓毒症患者APACHE-II评分;入院48小时内进行24小时动态血压监测。依据APACHE-II评分将脓毒症患者分为APACHE II评分19组和APACHE II评分≤19组,比较两组一般临床指标、实验室检查和24-hours SBP.24-hours DBP、SBPV、DBPV、PP、Dipper百分比。应用Spearman相关性检验分析BPV和炎症指标相关性。依据住院死亡率将脓毒症患者分为存活组和死亡组,根据脓毒症患者预后进行单因素分析,针对单因素分析有意义的指标,进行多因素Logistic回归分析,分析影响脓毒症患者预后的独立危险因素,绘制受试者工作特征曲线(ROC),并计算曲线下面积,分析判断脓毒症患者预后指标的价值。所有资料均由SPSS17.0统计软件进行数据处理,以p0.05为差异有统计学意义。 结果:1APACHE II19脓毒症患者,SBPV、DBPV、非勺型百分比.COR. LAC、PLT和GLU较APACHE Ⅱ19患者明显增高(P0.05); PCT、WBC、 Cr、PaO2、CRP、ACTH和TNF-a无显著差异(P0.05)。 2相关性分析表明,SBPV、DBPV与APACHEⅡ评分显著相关(r=0.732,P0.01; r=0.762, p0.01), SBPV、DBPV与COR具有相关性(p=0.018,r=0.318;p=0.008,r==0.353),与TNF-α、IL-10.PCT无明显相关性(P0.05)。 3Logistic回归分析显示,SBPV、DBPV、APACHEⅡ评分和LAC是患者预后的独立危险因素。ROC分析显示,DBPV、SBPV、APAHEⅡ评分和LAC曲线下面积分别为0.890、0.746、0.831和0.915. 结论:1脓毒症患者SBPV、DBPV和非勺型百分比增高;2DBPV和SBPV可作为判断脓毒症患者预后的独立危险因素;3.脓毒症患者DBPV和SBPV与炎症指标无关。
[Abstract]:Objective: to observe the changes of blood pressure variation rate in sepsis patients and its relationship with the critical degree of sepsis, to observe the relationship between blood pressure variation rate and endocrine, inflammatory index TNF- 伪 IL-10, and to explore its possible mechanism. Methods: from November 2011 to April 2012, 89 patients with sepsis in emergency center of Tianjin Medical University General Hospital were selected. The general clinical indexes, vital signs, blood routine examination, liver function, renal function and PCT, electrolyte were recorded within 24 hours after admission. The APACHE-II score of sepsis patients was calculated by collecting the worst clinical indexes and the ambulatory blood pressure was monitored within 48 hours after admission.
