颅脑损伤早期外周血中性粒细胞氧化爆发和吞噬功能的研究

【摘要】:目的:研究颅脑损伤患者早期外周血中性粒细胞的氧化爆发及吞噬功能变化。 方法:选择12例颅脑损伤患者(颅脑损伤组,TBI),12例骨折患者(创伤控制组,TC)及10例健康者(健康对照组,U),采用流式细胞术、Westernblot等方法分别于伤后6h、12h、24h、48h、72h、1w及2w检测外周血中性粒细胞氧化爆发强度,吞噬率,自由基,NF-kB,gp91phox,COX-2,iNOS,及血浆TNF-α,IL-6和CRP的变化情况。 结果:TBI组较U组在各时间点中性粒细胞氧化爆发明显增强(P<0.05),,且高于TC组;TBI组中性粒细胞的吞噬率在各时间点均明显降低,而TC组在24h-72h降低(P<0.05),TC组中吞噬率在伤后1w可恢复正常范围,TBI则在伤后2w仍处低水平吞噬活性状态;除外TC组在2w时,其余时间点TC和TBI组中性粒细胞自由基均明显增加(P<0.05),且颅脑损伤越重,增加越明显;伤后24hTBI组和TC组中gp91phox,COX-2和iNOS的表达均增加(P<0.05),且TBI组较TC组增加更显著,而NF-kB的增加则两者相似;细胞因子TNF-α,IL-6以及CRP在TC组1w后可趋于正常,而在TBI后2w内仍处于高水平(P<0.01)。 结论:颅脑损伤早期外周血中性粒细胞氧化爆发作用增强,而吞噬功能下降。
[Abstract]:Objective: to study the oxidative burst and phagocytosis of peripheral neutrophils in patients with craniocerebral injury. Methods: twelve patients with craniocerebral injury (12 patients with, TBI), fracture in craniocerebral injury group and 10 healthy subjects with, TC) in trauma control group) were treated with flow cytometry and Westernblot respectively at 6 hours after injury, 12 hours after injury and 24 hours after injury. The oxidative burst intensity, phagocytosis rate, free radical, NF-kB,gp91phox,COX-2,iNOS, plasma TNF- 伪, IL-6 and CRP of peripheral blood neutrophils were measured for 1 week and 2 weeks. The phagocytosis rate of TC group returned to normal at 1 week after injury, while the phagocytic activity of TBI was still low at 2 weeks after injury. The neutrophil free radicals in TC and TBI groups increased significantly at the other time points (P < 0. 05), and the more severe the craniocerebral injury, the more obvious the increase. The expression of gp91phox,COX-2 and iNOS in 24hTBI group and TC group were increased after injury (P < 0. 05), and the expression of gp91phox,COX-2 and iNOS in TBI group was significantly higher than that in TC group, but the increase in NF-kB was similar. The cytokines TNF- 伪, IL-6 and CRP tended to be normal after 1 week in TC group, but remained at high level within 2 weeks after TBI (P < 0. 01). Conclusion: in the early stage of craniocerebral injury, the oxidative burst of peripheral blood neutrophils is enhanced, but the phagocytosis is decreased.
