长春地区汉族青少年Ricketts头影测量分析方法全部指标正常值的建立

【摘要】:实验目的:建立长春地区汉族青少年正常牙合Ricketts头影测量分析方法全部指标的正常值,并进行男女差异性的比较研究,填补长春地区现有的Ricketts头影测量分析方法测量数据的不足,为该地区临床应用Ricketts头影测量分析法进行病例诊断、矫治设计及疗效预测提供参考值;与其它头影测量分析方法相比,Ricketts分析方法有其独特的优势,即生长预测和预后判断,有利于正畸医生为患者提供更全面、更精细的矫治方案,提高患者的矫正成功率,减少复发率。 实验方法:在长春市各中小学进行汉族青少年个别正常牙合的普查,选择标准:①籍贯是吉林省长春地区的汉族青少年;②牙齿健康,无明显的龋坏、缺损;③个别正常牙合(除第三磨牙外,牙齿数目正常;磨牙关系:中性关系,尖牙关系:中性关系,前牙区的覆牙合、覆盖正常,,咬合关系良好;④软组织正貌、侧貌基本协调、唇部肌肉形态、功能正常;⑤无颞下颌关节疾病;⑥无偏侧咀嚼、无夜磨牙等不良的口腔习惯;⑦无颌面颈部的外伤史;⑧未接受过正畸治疗或者正颌手术史;⑨无全身系统性疾病。筛选出符合实验要求的恒牙早期个别正常牙合青少年65例:男,30例;女,35例,(平均年龄:男:14.07±1.72,女:14.34±1.43)。在自然头位下拍摄头颅定位侧位片,应用dolphin17.0软件测量Ricketts分析方法各项指标,测量结果经SPSS17.0软件处理后,以均值±标准差(X±S)的方法建立了Ricketts分析方法各项测量指标的正常值范围;并通过t检验的方法比较各项测量指标间男、女差异性,P0.05有统计学意义。 实验结果:得出长春地区汉族青少年正常牙合Ricketts分析法全部指标的正常值;Ricketts分析法统计结果显示:除A点突度、上颌深度角、下颌体长度、前颅底长度性别差异有统计学意义外,其他各项指标性别差异均无统计学差异(P0.05)。 实验结论:本研究所建立的长春地区汉族青少年正常牙合Ricketts分析方法各项测量指标的正常值可用于长春地区汉族青少年正畸患者的临床诊断、矫治设计和预后的分析。长春地区汉族青少年Ricketts分析方法显示:牙性因素指标、牙与骨相互关系因素指标以及美学因素指标性别差异均无统计学意义;骨性因素指标中A点突度;面部相对于颅骨关系因素指标中上颌深度角;颅骨内部结构因素指标中下颌体长度、前颅底长度,测量值均为男性女性,性别差异有统计学意义,因此在临床正畸诊断、设计、分析中应考虑性别差异。
[Abstract]:Objective: to establish the normal value of all the indexes of normal occlusal Ricketts cephalometric analysis method in Changchun Han adolescents, and to make a comparative study on the difference between male and female, so as to fill in the deficiency of the existing Ricketts cephalometric analysis methods in Changchun area, and to provide a reference value for the clinical application of Ricketts cephalometric analysis in case diagnosis, correction design and curative effect prediction. Compared with other cephalometric analysis methods, Ricketts analysis method has its unique advantages, that is, growth prediction and prognosis judgment, which is helpful for orthodontists to provide patients with a more comprehensive and precise corrective scheme, improve the success rate of correction and reduce the recurrence rate. Experimental methods: a general survey of individual normal occlusal among Han adolescents was carried out in primary and secondary schools in Changchun, Jilin Province. The selection criteria were as follows: (1) Han adolescents from Changchun, Jilin Province; (2) healthy teeth, no obvious caries and defects; (3) individual normal occlusal (except the third molar, the number of teeth was normal; molar relationship: neutral relationship, overbite in anterior occlusal area, normal overbite, good occlusal relationship; (4) the appearance and side appearance of soft tissue were basically coordinated, the shape and function of lip muscle were normal; (5) there was no TMJ disease; (6) there was no bad oral habit such as unilateral mastication and no night molars; (7) there was no history of maxillofacial and neck trauma; (8) there was no history of orthodontic treatment or orthodontic surgery; (9) there was no systemic disease of the whole body. 65 cases of normal occlusal adolescents in the early stage of permanent teeth were selected: male (30 cases) and female (35 cases) (mean age: 14.07 卤1.72, female: 14.34 卤1.43). The lateral cephalometric films were taken in the natural head position, and the indexes of Ricketts analysis method were measured by dolphin17.0 software. After the measurement results were processed by SPSS17.0 software, the normal value range of each measurement index of Ricketts analysis method was established by means of mean 卤standard deviation (X 卤S), and the differences between male and female were compared by t test, P 0.05. Results: the normal values of all the indexes of normal occlusal Ricketts analysis were obtained in Changchun Han adolescents, and the statistical results of Ricketts analysis showed that there was no significant difference in other indexes except point A protruding degree, maxillary depth angle, maxillary body length and anterior skull base length (P 0.05). Conclusion: the normal values of Ricketts analysis method for normal occlusal of Han nationality adolescents in Changchun can be used for clinical diagnosis, correction design and prognosis analysis of orthodontic patients of Han nationality adolescents in Changchun area. The Ricketts analysis method of Han nationality adolescents in Changchun showed that there was no significant difference in dental factors, tooth-bone correlation factors and aesthetic factors, A point protrusions in bone factors, maxillary depth angle in facial relative skull factors, and no significant differences in tooth sex factors, tooth-bone correlation factors and aesthetic factors. In the internal structural factors of skull, the length of mandibular body, the length of anterior skull base and the measured value were all male and female, and the gender difference was statistically significant, so the gender difference should be considered in clinical orthodontic diagnosis, design and analysis.
